After accomplishing this form, mail it to Philippine Air Force Personnel
Management Center, Col Jesus Villamor Airbase Pasay City together with the
following: Original NSO Birth Certificate and original copy of Transcript of Record or
REPRODUCTION / PHOTOCOPY OF THIS FORM IS AUTHORIZED
 certification from the registrar of last school attended (all documents shall become
PAFPMC property).

QUALIFICATIONS:

• Male or female, natural born citizen of the Republic of the Philippines

• Must have a Baccalaureate Degree from any government-recognized institution on or before the date
of filing of his or her application for admission
• Must have a height of at least 162.5 cm (5’ 4”) for male and 152.4 cm (5’ 0”) for female
• Not less than 20 y/o nor more than 24 y/o upon admission for training on 15 March 2010 (Born
between 15 March 1986 and 15 March 1990)
• Single, never been married nor have any legal obligation to support a child or children
• Physically, mentally and psychologically fit for military training
• Of good moral character

• Applicants who passed the written examinations belonging to the top 100 will be notified to
report at PAFPMC for their Special Written Examination (SWE) and Physical Fitness Test (PFT). Only
those who submitted all the required clearances and other documents as enumerated below will be
allowed to take the SWE.

• Those who will pass the SWE will be required to take and pass the PFT and those who passed
the SWE and PFT will be issued Authority for Physical/Medical Examination.
