LIABILITY RELEASE

RICK SAMMON AND SAMMON & SAMMON LTD. (SAMMON &

SAMMON LTD. FROM THIS POINT FORWARD) welcomes you to our photo tours
workshops. The following guidelines must be reviewed and accepted prior to beginning
the workshop with us. Please sign where indicated, acknowledging your agreement to
these provisions:

AGREEMENT
(1) Loss or Damage to Personal Property of a Workshop Participant:
Sammon & Sammon Ltd. or any of its affiliated companies, owners, employees or
agents shall not be responsible for the loss or damage to a customer’s personal property.
Customer brings any of their property to the workshop at their own risk.

(2) Injury:
All participation is done at the risk of the participant, and Sammon & Sammon Ltd. is
not responsible for any personal injury arising wherefrom. Participant attests that he/she
is in good medical condition and physically capable to perform activities consistent with
a photography workshop.

The Undersigned hereby releases, discharges and forever acquits, Sammon &
Sammon Ltd., its respective owners and employees of and from any and all liability,
claims, demands, actions and causes of action whatsoever, arising out of or related to any
loss, damage or injury, including death, that may be sustained by the undersigned or any
participant in, person at, or to the property of the undersigned while either participating in
or being present at the workshop including, but not limited to, those injuries and damages
caused by negligence whether it be active or passive, gross, wanton, or ordinary on the
part of Sammon & Sammon Ltd and employees arising from the undersigned while
participating in or being present at the workshop.
By signing this Agreement, the customer understands and agrees that he/she
waives his/her rights and the rights of his/her heirs, guardians, administrators and
executors to all claims arising out of their participation in the workshops and any
programs with Sammon & Sammon Ltd. including but not limited to personal injury,
theft, or loss of personal property.

I also understand that trip cancellation insurance is highly recommended, as is a fully

refundable airline ticket.
