We, Ivan D. Reyes, King James D. Cruz and Gino Eleno Carlo V. Mendoza, students of Mapua
Institute of Technology – Makati, are currently taking the course Capstone Project under the
instruction of Dr. Torres and advising of Prof. Elcid A. Serrano, as a partial fulfillment of the
requirement for the degree in BS Information Technology.

Our approved topic for this Capstone Project is iCSAD: Student Infraction and Activity
Monitoring System, it is a Web Application developed in ASP.Net intended for the use of Center
of Student Activities and Discipline (CSAD) - Makati in terms of managing and monitoring
students infraction and student organization activities in Mapua Makati.

In this regard, we would like to ask for your permission if it would be possible for us to connect
to the school’s database so that we would be able to use student’s details such as infraction
incurred and unsettled infractions as well, we would be also needing some other information that
might be helpful in the development of our Capstone Project for the CSAD Makati Office
through the database of the school. Database Schema or the Dummy MDF will also be useful for
us in patterning the database of the school if direct connection cannot be permitted due to
security reasons.
Should you approve our request, we would like to meet with you in your office during your most
convenient time to discuss the necessary and additional details of our capstone project and our
request.
