enter your choice of Option A or Option B in the space provided at the bottom of this form,
and date.

As an employee of the Minidoka County Joint School District, I understand that I am eligible for
worker's compensation benefits as outlined in Minidoka County Joint School District Policy
#807.13.
Option A. Worker's Compensation Benefits Only
Employee may elect to enter a period of temporary leave of absence without pay and receive
worker's compensation benefits only.
Option B. Worker's Compensation Benefits Supplemented by Accrued Sick Leave.
Minidoka County Joint School District will allow the employee to use his/her earned sick leave
to supplement the workman's compensation benefits to maintain his/her regular salary insofar as
the combined total of worker's compensation benefits and sick leave benefits do not exceed one
hundred percent (100%) of the salary to which the employee would otherwise be entitled. For
those employees eligible for district benefits, the district will continue to pay the employees life,
health, and dental insurance premiums as if the employee were still physically at work and will
deduct from his/her pay the same amount of self paid premiums as would be with held if
physically working, provided funds are available to the employee from accrued sick leave.
Arrangements for payment of benefits and liabilities beyond that level are to be made with the
District Payroll/Benefits Department.
If the employee is a member of the Public Employee Retirement System (PERS), he/she will
receive one month of service credit with PERS for each month he/she is receiving worker's
compensation benefits combined with and limited to the amount of accrued sick leave used.
The employee's income maintenance and retirement credit will continue as long as he/she has
approved sick leave available, unless the employee has been laid off or resigned due to his/her
inability to return to the position and perform assignments as required by the job description. If
the employee has depleted all available approved sick leave then Option B becomes unavailable.
I have read Option A and Option B and the policy in their entirety, and have elected Option: .
