Objectives of the Study:

After 2 days of community immersion we will be able to:

• Apply the things that we have learned during our related learning experience in the health center

and in the community like conducting a physical assessment.

• Conduct an effective interview ad to have a personal contact with our client and foster parents.

• Enhance our knowledge and skills prior to our concept (fluids and electrolytes).

• Conduct an efficient health teaching on the people of the community.

• To develop teamwork, work ethics and time management during the community immersion.

• To be able to formulate 3 NCP and 5 Prioritized Nursing Diagnosis prior to our client’s condition.

Description of the Community Profile.

The client’s house is located in luta Sur, Malvar Batangas near the health center. The house of the

client is enough for them. They have proper restroom, bedrooms and etc. The community surrounding

is clean, refreshing and well organized. There are many houses around the area that is made up of

concrete and wood. The people n the community was hospitable, friendly and patient. The

community helped us made our program successful. They help each other in times of difficulties.

CHAPTER 2

A. Biographical Information

Client a 69 yrs. Old female, Filipino and a roman catholic. She’s married to M. C. is a widow

with 6 children. She is a plain housewife. She was born on Jan. 19, 1941 in Luta Sur, Malvar,

Batangas and presently residing in the place. Her source of reliability of information is her

daughter S.C. Date of interview was Sept. 9, 2010.

B. Chief Complain

“Ala eh, lumalaki ang bukol sa leeg ko, simula pa noong Marso 20, 2008,” as stated by the client.

C. Present History
 Client feels difficulty in swallowing and difficulty in breathing due to her condition. Client also

verbalized that her goiter started in 2008 she was given medications for this. She has a mass on

her neck. Client feels uncomfortable and uneasy.

D. Past history

Client claims that she has no childhood injuries had chickenpox that was treated at home.

Common illness includes fever, flu and cough.

E. Allergies

Client has no allergies on foods, drugs and any environmental factors.

Immunization Status.

Analysis:

Client has a complete immunization except for HEPA B. The BHW was the one who gave the

vaccination to the client.

Current Medication:

Aspirin 500 mg once a day.
