Outstanding Project Award OR Individual Achievement/Service Award 
2011 NOMINATION FORM 
 
 
Purpose: 
The  Eden  Prairie  Chamber  of  Commerce  wishes  to  recognize  both  teachers  and  staff  in  our  Chamber 
member schools for instilling the importance of its commitment to learning, a focus on positive values and 
the empowerment of students in useful roles in the community.  These attributes represent a number of 
developmental assets.  This award also rewards creativity, enthusiasm, commitment, and development of 
positive learning environments.  The Eden Prairie Chamber of Commerce supports and fosters interaction 
between the schools, the residents and the business community.   
  
Award: 
The  Golden  Apple  Awards  recognize  outstanding  contributions  to  education  in  our  Chamber  member 
schools.  The Eden Prairie Chamber of Commerce solicits nominations to recognize contributions in one of 
two areas: 
 

Outstanding  Project:    A  Golden  Apple  Award will  be  presented  for  an  exemplary  project  that  has 
benefited  the  students  or  has  had  a  lasting  impact  on  the  students/school/district/community 
during the past school year. 
 

Individual Achievement/Service Award:  A Golden Apple Award will be presented to an individual 
for  an  exemplary  contribution  to  education.    The  individual  may  be  recognized  for  his  or  her 
commitment to student development over the past year or through the long‐term service to his/her 
educational institution.    
 
Eligibility: 
All staff at our Chamber member schools (K‐12 & Post Secondary), including teachers and support staff are 
eligible for this award. 
 
 
Nomination Instructions:

1. Nominations may be submitted by staff, students, business or community people. 
2. Complete  the  nomination  form  with  the  necessary  detail  to  allow  the  Chamber’s  Recognition 
Committee to make an award decision.  Attach additional sheets if necessary. 
3. Return the  nomination form to Eden Prairie Chamber of Commerce, 11455  Viking Drive, Suite
