ORDER PLACEMENT AND PROCESSING

(March – April 2011)

During the transition period of March through April 2011, Unicity Health Private Limited has
established a process to allow distributors to order product and start building their teams. The
process detailed below is available to specific team leaders. Only identified team leaders are
approved to use this process. If you would like to be approved, please send an email to
unicityindia@unicity.net.

Order Placement and Fulfillment Process

Step 1 - The team leader collects applications, order forms and payments including VAT from
the team members.

 If the person has not registered as a distributor, the application form must be
completed and the order may be included on the application form.
 If the distributor registered online, only a completed order form is required.
 Payment may be accepted (wire, direct deposit, check, cash) as deemed appropriate
by the team leader.
 A simple invoice should be made available to the distributor making the payment.
 A copy of the application or order form should also be made available to the
distributor.
 After March 15, each application must be accompanied by a registration fee of 900
rupees. The registration fee should be listed separately on a product order line.
 For Manager orders (1,000 points) the registration fee is waived.

Step 2 - The team leader prepares a spreadsheet with the distributors’ Unicity ID numbers,
names, products ordered, PV related to order and payment amount.

Step 3 - The team leader sends the completed spreadsheet, applications, and order forms to
Unicity Global Headquarters in the United States. It is recommended that documents be faxed
to Unicity or scanned and emailed--including the spreadsheet. The email address is
unicityindia@unicity.net. The fax number is US number 1-801-226-4499.

Step 4 - The team leader sends a consolidated payment to a Unicity bank account. There are
two bank options for the team leader. Detailed information is provided on Attachment 1.

Product Order Options

A summary of product order options, packages, pricing and points is found on Attachment 2. It is
recommended that the team leader use this information to help team members choose the best order
for their needs.

Commission Payments
Commissions and bonuses for product ordered during March will be paid by April 20th. Commissions
and bonuses for product ordered during April will be paid by May 20th.
