When in doubt call your immediate
supervisor or FSM. Before entry into a
confined space, an assessment shall be
performed to determine if the space
shall be identified as a Permit Required
confined space. All employees must
have received training on confined
space as well as understand their
individual responsibilities. Verify all
aspects of confined space permit are in
place if applicable. Air monitoring will
be provided prior to entry and then
continuous. 4-Gas Monitor will be
verified for proper calibration dates.
Acceptable atmosphere:

 Oxygen content >19.5% & <23.5 O2


 Flammable gases & vapors < 10% LEL
 Toxic gases and vapors < the PEL for
known containments

Assemble PPE, Tools and Rescue

equipment swing radius shall be
guarded.
F. Employees using powered compaction
equipment (jumping jacks, tampers,
etc.) shall utilize metatarsal guards.
G. A competent person shall inspect the
excavation/ trench prior to each day’s

activities or any change in conditions.
During trenching operations, a certified
trench box, sloping, or engineered
shoring shall be used. Access/ egress
(ladders) shall be provided within 25
feet of all employees. Air monitoring
shall be done on a daily basis. If a hoe
ram is utilized hearing protection and
flying object protection shall be
provided. Protective fencing shall be
installed for protection of employees
near the excavation and prevent
unauthorized entry into the area.
Employees shall be aware of the slip/
trip hazards present during wet/ icy/ or
snowy conditions. Excavations of earth
material to a level not greater than 2
feet (.61 m) below the bottom of a
shield shall be permitted, but only if the
shield is designed to resist the forces
calculated for the full depth of the
trench, and there are no indications
while the trench is open of a possible
loss of soil from behind or below the
bottom of the shield. Dewatering shall
occur as necessary and the excavation/
trench inspected by the competent
person prior work beginning. All spoils
shall be at least two feet from the edge
of the excavation.
H. Employees shall wear high visibility
vests. All back up alarms on equipment
must be in place and serviceable.
Spotters shall be used as necessary for
equipment positioning. A flagman shall
be used as necessary at the access/

the workplace:

Good personal hygiene will be

exposure. Eating, drinking, use of
tobacco products or applying cosmetics
will not be done in areas where there
is dust containing crystalline silica.

If possible, employees will shower and


change into clean clothes before
leaving the worksite to prevent
contamination of cars, homes, and
other work areas.
