Learning License:

1. Residence Proof :

Attested by Gazetted officer/ Notary copy of any of the following:

a. Ration card with affidavit

b. Electricity/Tel. Bill/House Tax receipts/ Pass Book of National bank with affidavit
c. Life Insurance Policy
d. Passport
e. Voter Identity card
f. Pay slip issued by any office of the Central/State or a local body with Identity Card
g. The consent from biological parents can be treated as residence proof, if parents
enclose their residence proof (For minor 16 to 18 year).

2. Age Proof:

Attested copies of any of the following:

a. School Certificate (X Board)

b. Passport
c. Birth Certificate
d. Certificate from Central/State Govt. or a local body
e. PAN card

Other authentic proofs accepted by M.L.O.

f. Form No.2 : Application for learner license with passport size photograph.
g. Form No.1 A : Medical certificate –It is required in case of commercial licenses or age
above 50 years for private non/commercial vehicles having passport size
photograph.
h. Form No.1: Self declaration of medical fitness.

Please bring the originals of the attested copies being presented as proofs.

Eligibility for Permanent License and Additional Endorsement of Driver’s License

(AEDL) cases:

• You should have a valid learner license.

• You should apply after 30 days and within 180 days from the date of issue of learner
license.
• You should be conversant with the vehicle systems, driving, traffic rules and
regulations

Documents Required:

i. Original valid learner license with Online Appointment Confirmation Slip.

ii. Application in Form No.4
iii. One recent passport size photograph
 iv. Form No.5 in case of commercial license (for Renewal or Change of Address
Services).
v. A vehicle from the category in which you are applying for the license.
vi. Your original license in case of endorsements of categories.

Remember to bring your original documents along with the above.

Documents required for renewal of Driving License:

For Private License

1. Application in Form No.9

2. 3 copies of your recent passport size photograph
3. Driving license
4. Proof of age and residence
5. Form No. 1
6. Medical form No.1 A if age is more than 50 years.

For Commercial License

1. The above documents as used in Private License

2. Medical form No.1 A in all cases
3. Driver Refreshing Training Certificate in case of HMV
4. Traffic Challan Clearance

Procedure to get a Duplicate License:

1. Application in Form LLD

2. FIR of the lost license
3. Challan clearance report from Traffic Police (in case of Commercial license renewal).
4. Address Proof / Age Proof
5. Form No.1/1 A with 2 photographs

Procedure to get a International Driving Permit (IDP):

1. Form 4 A and 1 A
2. Valid License
3. Address Proof
4. Valid Passport and Visa in case of online visa the same may be attested/verified from
embassy
5. Rs. 500 fees for IDP
Change of Address:

1. Form 33
2. Address Proof / Age Proof.
3. Original License
