Madam:

This refers to the Contract of Agreement you entered into on January 25, 2010 as a
service provider to conduct a Baseline Study to generate a Comprehensive Sex/Gender
Disaggregated Data.

Per contract, you are to conduct a survey and give a complete report as stipulated
thereat in four months beginning January 25, 2010 and ending on May 25, 2010.
Moreover, the Final Report must contain the data analysis of the study which is vital to the
on-going projects of the Women Micro-enterprises, the LGU Officials and the Employees.

Please bring your attention to the abovementioned dates. Those dates are being
highlighted because other programs and projects of the stakeholders are depending on the
outcome of the job you contracted. In fact, the LGU Officials and the Employees have
already proceeded with their programs without the benefit of the result of the study.

In the meeting on February 11, 2011, you presented to the stakeholders only an
incomplete report wherein your overdue Analysis/Final Report was expected. In addition,
no rectification has been done yet on the complaint raised regarding the manner you
treated the survey forms to some of the respondents contrary to survey methodology we
emphasized to you during the training. Furthermore, the LGU officials and employees who
were respondents of the survey and were present during the presentation, were surprised
that their answers were not included in your report. It was also found out that you were
using different point of references to the three pilot municipalities.

In generalization, the stakeholders are not satisfied with the progress and the
scientific veracity of your work at-hand, notwithstanding the 73% of the contract price or a
total of P457,518.75 payment you have collected from us.

In this connection, please correct the errors starting from the data gathering, then
reflect all the answers given in the survey forms and finally, give us the Analysis/Final
Report.

Inasmuch as you have been given too much time, we insist that you deliver us the
Analysis/Final Report, as well as the survey sheets (secondary data) within fifteen days
from receipt of this letter otherwise, we shall proceed with the legal remedies attributed
to an aggrieved contracting party.
