Brooks Family Daycare

CONTRACT

I am a certified elementary school teacher who is


providing home daycare.

Payment of one week in advance is required to


begin child care services or to hold a space.
This is a non refundable payment if you
choose not to use my services. This advance
payment is used as the final week of service
payment.

The rate for the care of one not school age is


$125.00 per week. Part-time care is provided if
availability allows. The fee is due each Monday for
the week that follows. This includes meals and
learning time supplies. Diapers, wipes, and
formula/baby food to be provided by parent. There is
a $5.00 fee for each day after Monday that payment
is not received. A late pickup charge of $1.00 per
minute will be charged after 5:30 p.m. The
occasional unexpected and un-preventable lateness
will not be charged. Chronic lateness and abuse of
this policy will result in being charged. Phone notice
must be given if you are going to be late.

The rate for Green Tree Elementary students


before/after care is $60 a week. There will be an
additional charge of $5.00 per day for kids attending
daycare during: holidays, school breaks, and in
service days. Please be sure to discuss bussing
procedures with me.
Daycare is closed and no care is provided on the
following holidays and will be paid holidays by the
daycare clients with no exceptions:
• New Year’s Day (if it falls during a week day)
• Martin Luther King day (observed)
• Presidents Day (observed)
• Memorial Day
• July 4th (if it falls during a week day)
• Labor Day
• Thanksgiving Day and the day after
Thanksgiving
• Christmas Eve and Day (if it falls during a week
day)

Days the parent(s) choose to keep the child home


are to be paid. Days the child care provider cannot
provide care are to be deducted from the weekly
rate. ($25 if full time $12 before/after school)

ATTENDANCE
Hours of operation are Monday thru Friday, 6:30am –
5:30pm. Payment of child care services will start at
the time contracted for and cannot be adjusted for
late arrival or early pickup. Please call if your child
will not be attending for the day. You are welcome in
my home anytime during the business hours. Please
be aware of naptimes.

VACATION
One week vacation per contracted (start date – start
date) year can be taken without having to pay and
the child’s space will be held. Vacation must be
taken during 5 consecutive days. All other vacations
during the same contract year will require the full
rate to maintain your spot in daycare.

The provider will give parents one month advance


notice for her vacation. It is the parent’s
responsibility to arrange for alternative care during
the provider’s vacation. There are no fees charged
during the provider’s vacation. I typically take 1
week around the Christmas holiday and/or possibly
one week during the summer.

A substitute adult will be arranged for any time


during the scheduled daycare time that the provider
must attend to personal business away from the
home. You will be given advance notice (when
possible) of who the substitute will be and during
what hours the substitute will be in charge.

ILLNESS AND MEDICINE


I will provide care for minor illness such as a minor
cold. The provider will administer medications
provided by the parent if the Medical Consent form is
completely filled or if a copy of the doctor’s
prescription is provided. However, I will not care for a
child who has unexplained diarrhea, vomiting, and/or
has a temperature of 100°f or above. Please read the
sick child policies for more details.

The provider will make the final decision as to care of


a sick child. Any child who has a contagious illness
will not be taken care of by the provider and should
not be brought. A 50% discount is given if a child
misses a full week of care due to illness with a
doctor’s note or proof of medical exam.
If a child becomes ill or is injured at the family day
care home and the parent or emergency contact
cannot be reached, the physician listed on the
Emergency Form will be contacted.

The parent or legal guardian is responsible for any


and all costs incurred for medical care, and or
transportation provided during the medical
emergency. Please provide a copy of your insurance
card to be kept on file and notify provider when
changes occur.

Scratches and scrapes are inevitable when children


play. These injuries will be treated with antiseptic
and Band-Aid. With anything more serious, the
provider will inform you at once.

SICK CHILD POLICIES


It is very important that we attempt to control the
spread of communicable diseases. We would
appreciate your consideration in following these
guidelines:
1. Students should be fever-free (at 98.6°f without
the influence of fever reducing medication) for 24
hours before returning to daycare.
2. Students should be free of contagious coughing.
3. Students should remain at home for 24 hours after
taking the first dose of an antibiotic for an infection.

MEALS
Infants will be fed formula or breast milk. Both to be
supplied by the parents. Baby food is also to be
provided by the parents. Full day children will be
served breakfast, an AM snack, lunch, and a PM
snack. Before/After care children will be served
breakfast and a PM snack on school days. Please
refer to the schedule for these times. These meals
will be served by the provider and are included in the
fees. These meals will be part of a well balanced diet;
please do not send gum, candy, or “junk food”
without discussing it first with the provider. A menu
will be provided. If your child requires a special diet
due to allergies, medications, and/or cultural or
religious beliefs it will be the responsibility of the
parent to provide a well balanced lunch and snack for
their child. Dinner meals are available at an
additional cost when you will be late.

DISCIPLINE
Discipline consists of “time-out”, redirection, and
gentle, but firm limit-setting. Corporal punishment
(i.e. Spanking, hitting) is NEVER an option. If a child
cannot control their behavior, within reason of their
cognitive development, the parent will be notified
first. If the incidence of behaviors continues, the child
will be asked to find alternate daycare immediately.

MISCELLANEOUS
The child may bring one or two comfort items from
home for naptime (i.e., stuffed animal, pacifier, small
blanket or pillow). The child is not permitted to bring
toy weapons – guns, knives, water guns, etc. Please
discuss with your child that any play toy they bring,
they should be prepared to share with the other
children.
For field trips (typically the Zoo, Monkey Joe’s, etc)
you are asked to provide any spending money or
admittance fees. You will be given 1 week notice of
field trips. Any money not used will be returned to
you.

TERMINATION
The provider may terminate child’s enrollment
effective immediately if the child’s behavior
threatens the physical or emotional well being of one
or more of the other children at the family daycare
home. Provider will refund 50% of any remaining
whole days of the weekly fee paid by the parent for
the time that child care services will not be offered.

The provider may terminate child’s enrollment


effective immediately if the parent is verbally or
physically abusive, or threatening to the provider.
Two weeks notice is required for termination of child
care by either party if under “normal circumstances.”
If a parent does not wish to give prior notice of
termination, then two weeks’ rates must be paid in
full. One of the weeks can be paid using the initial
spot hold deposit.

Chronic late payment of services is cause for services


to be terminated without notice. There will be a
$25.00 charge for all personal checks returned NSF
plus any additional charges incurred to myself and/or
by my bank (typically $30).

**The first 10 days are probationary period for the


provider, parent and child. This agreement may be
terminated at any time during this period.

**I keep a record of your payments and will give you


an end of the year tax year statement. If you need a
weekly receipt please let me know.

FOR THE PARENTS


Parents are welcome to come by anytime during the
day.

Signatures signify agreement to the above terms and


conditions listed in the contract.

As this is a Family Day Care Home, I feel it is most


important that you and I are able to discuss any
problems that might come up. I want your child to
feel comfortable and happy in our home.
