Registration
Your registration fee includes admission to all symposium sessions and exhibits, daily coffee breaks, and the symposium
reception on Thursday evening. Take advantage of specially reduced fees by registering in advance.

How to Register
Complete the enclosed registration form and do one of the following:

• Mail your form and check (payable to Quintessence Publishing Co, Inc) for the registration fee to Quintessence

• On-site registration will be offered only on the basis of space availability and thus cannot be guaranteed. Early advance
registration is strongly recommended.
The deadline for registering for the symposium is May 25, 2011.
Cancellations/Refunds
Cancellations made in writing and received by Quintessence before May 25, 2011 will be refunded in full. For cancellations
received on or after May 26, 2011 but before the start of the meeting, a cancellation fee of US $75 will be charged. No refunds
will be made after the symposium begins on June 9, 2011.
Lost Badges
Your badge represents your ticket to admittance to all ISC lectures. If you lose or misplace your badge, there will be a
replacement charge of $300; there are no exceptions to this policy. You must wear your badge each time you wish to enter
the lecture halls.
Registration Desk
Your symposium packet will await you at the symposium registration desk. The desk will be open from 1:00 pm to 6:00 pm on
Wednesday, June 8, and from 6:30 am on Thursday through Saturday. Please note that on-site registrations will be accepted
only if room capacity allows.
Hotel Reservations
Hotel rooms have been blocked for symposium participants at the Sheraton San Diego Hotel & Marina. Reservations at the
special symposium rate will be accepted until the cut-off date, May 16, 2011, or until the room block is sold out, whichever
comes first. Complete the hotel reservation form in this program and mail it with your registration form to Quintessence, or
call the hotel reservation desk direct at 877-734-2726. Be sure to indicate the name of the symposium to receive the special
meeting rate.

participants. To make your reservations, please complete this form and mail it with the registration form to Quintessence, or call the
reservation desk direct at 877-734-2726. (If you call the hotel, be sure to indicate the name of the symposium to receive the special
meeting rate.) If you plan to mail the form to Quintessence, please do not call the hotel for reservations, because your reservations
may be duplicated.
Please note that the reservation cut-off date for the special meeting rate is May 16, 2011 or when the block is sold out, whichever
comes first. Therefore, it is recommended that you reserve your room as early as possible. The hotel requires a one-night deposit to
confirm your reservation, guaranteed with a credit card. To change your reservation dates or to make a cancellation, please contact the
hotel direct. A 1-night deposit is refundable 10 days prior to arrival date; make sure you receive a cancellation number. Cancellations
made less than 10 days prior to arrival will result in the forfeit of your deposit. If you check out prior to your reserved departure
date, the hotel will charge you $100.
