l health assessments and Treatment plans. I served a caseload of an average of 3
5 SMI Adults, based upon a recovery model, utilizing a strengths and Solutions-f
ocused service crisis and treatment planning strategies.
I served the recovery needs of those in psychiatric, financial, housing, and voc
ational needs fulfillment. I was also required attend internal clinical meeting

Performed screenings and intakes, maintained full beds all times. Maintained pr
ogress reports on all male residents, who came to the agency through CPS and the
Juvenile Justice Systems. Faxed monthly progress reports to Case Managers, Soci

Services for all residents. I also maintained human resources: Created and maint

I supervised 6-8 Group Home Staff Members, and managed
the day to day running of one group home in North Phoenix.
I performed Human Resources responsibilities, including new
hire orientation and training, staff discipline, delegation of duties, and atten
ded weekly training meetings. I performed all new Adult Male Brain Injured resid
ents' intakes and maintained the completeness and privacy of their files. I deve
loped progress reports and treatment plans for each
Resident, maintaining professional continuity of care with Case Managers, Doctor
s, Therapists, and Probation Officers. I also maintained my assigned home accord
ing to all licensing standards. I observed staff and supervisory interactions at

Emergency Case Manager
Served the Recovery needs of homeless Adults and their
Children, most families afflicted by substance abuse of
their parents or guardians. Therefore, I addressed this issue as primary to the
financial recovery and welfare of these families. I led all homeless families t
hrough and intensive four month program, in which I met with each family on a we
ekly basis to create and monitor goals and their achievements. Within these meet
ings, I lent my support, and assisted parents with my knowledge of community res
ources, as well as the excellent resources within our campus community. I also a
ssisted with informal active listening and supportive responses, as well as offe
ring drug referrals, food boxes, and bus passes. Likewise, I provided 30, 60, an
d 90 day progress reports in a staffing format with superiors and Substance Abus
e Crisis Counselors, on behalf of each family on my assigned caseload. I also pe
rformed intakes on each new family assigned to my caseload.
03/2001-09/2002 Arizona Health Care
Program Manager
Duties included first Supervisory position, was manager of my assigned Long Term
Care House. I saw to clinical supervisions for each staff member on a monthly b
asis, reviewed progress notes, performed weekly staff training meetings, perform
ance incentives for improved performance, based on weekly training sessions, res
ponsible for assuring the house was fully staffed 24/7, and if no one was availa
ble for any given shift, I took and covered the shift myself. Being my first Sup
ervisory position, I put in countless overtime hours, there early and home very
late every day, and unable to attend to my own home personal matters, therefore,
the reason for the brief stay in this positon. However, I learned an incredible
amount about being a supervisor and improved my professionalism. I attended wee
kly training, full day training sessions at the corporate office every Thursday,
where in I learned more than I have ever learned at any other postion, irrespec
tive of my brief stay with this company.
06/1999-03/2001 ABS/Value Options, Phoenix, AZ
Adult SMI Case Manager
Responsible for Adult SMI caseloads of varying ratios, and levels of mental illn
esses. I performed liaison duties between clients and their families, doctors, t
herapists, and day program staff. I also assisted clients in grocery
shopping, transported them to medical doctor appointments when necessary, and pi
cked up their needed medications. I performed weekly home visits, monthly report
s, daily clinical meetings, weekly staffing meetings, alpha reports, treatment p
lanning, crisis plans, staffing client problems such as lack of medication compl
iance, drug abuse and dependence, and lack of treatment compliance problems man
agement. I was responsible for visiting clients in hospitals, jails, and crisis
centers, as well as, completion of legal
documents to petition or to amend petitions to provid evaluation and treatment f
or those who were involuntary and not wanting to be treated. I also developed an
d provided the resources for discharge plans, was cool under
crisis and fire, and perfected professionalism in managing competing priorities.
