Short Term Rehabilitation:
Renovated 23-bed Rehab Center to improve occupancy, desirability to younger cus
tomers, and RUG scores.
Improved Rehab Discharges to Home from 64% to 80% in 2009.
Developed relationship with Highland Hospital Joint Replacement Center.
Increased Medicare per diem to $438 in December 2009.
Realized total Medicare reimbursement $550,000 over budget for 2009.
Increased ultra RUG scores from 0 in November 2007 to 30% in December 2009.
Developed a new dining services program just for Rehab
Resident Care:
Changed dental hygienist position to improve customer service and dental care.

Improved dementia care adding programming, changing the activities staffing and
dedicating a separate room for day activities on the dementia unit.

Renovated one skilled nursing unit, adding amenities to make it more attractive
for private pay residents.
Reorganized dining staffing to allow one dining services staff member to be pre
sent on each unit for each meal, freeing nursing for patient care and feeding.
Facilitated a new hands-on dementia care program which was eventually expanded
to all units and involved staff members from all disciplines.
Improved DOH survey results from 7 deficiencies in 2008 to 4 in 2009.
Staff Development and Organization:
Recruited and mentored new Directors of Nursing, Rehab, and Recreation.
Implemented electronic medical records and electronic medication administration
Changed pharmacies in order to facilitate electronic medication administration
Changed to a new physician group (Strong Health Geriatric Group)
Raised the CMI from 0.96 to 1.15 during the latter half of 2009.
Combined two billing positions into one and allocated one former biller to prov
ide clerical assistance to MDS Coordinator, Nursing and Administration.
Changed employee newsletter from quarterly to weekly adding color pictures and
electronic distribution.
Obtained $15,000 in grant funding from Rochester Works to support EMR training.
