Adoption Contract

hereby agrees to adopt
__________________________________________________________________
_____________, as a companion pet. In consideration for the
Adoption fee of
_____________________________________________________________, and
other good and valuable consideration, Fallen Angels Pet Rescue
hereby accepts this offer.
For the consideration noted above, I agree to the following terms
of the Contract:
1. I will treat the dog as a household companion, not as an
outside pet, and I will provide him/her with humane care and
treatment, including proper food, water, shelter, attention and
exercise. I agree to give my new pet a reasonable adjustment
period to get used to his/her new home.
2. I will provide proper veterinary care, including yearly wellness
checkups and appropriate vaccinations and heartworm
prevention.
3. I will keep my pet under my control at all times, and will not
allow him/her to run at large without supervision.
4. I will return the pet to Fallen Angels Pet Rescue in the event I
no longer want or can no longer keep or care for the pet. I
understand that my adoption fee will not be refunded after 30
days from the date of the signing of this agreement.
5. I will not transfer ownership or physical possession of my pet
to anyone other than Fallen Angels Pet Rescue, other than for
purposes of boarding my pet.
6. I will permit Fallen Angels Pet Rescue to make an initial visit to
my home, and to also make periodic visits to my home during the
first 90 days following the signing of this contract.
I understand that should Fallen Angels Pet Rescue deem it
necessary for any reason whatsoever, I agree to return the pet
and my adoption fee will not be returned.

I understand that the dog has been spayed/neutered and that

his/her state of health is represented to the best knowledge of
Fallen Angels Pet Rescue; however, I have been given no
guarantees or promises regarding the health or temperament of
the dog, and I adopt the dog at my own risk. I agree to have the
dog examined by a licensed veterinarian within 10 days of
adoption. If upon such examination it is determined that the
health of the dog is not satisfactory to me, I may return the dog
only to Fallen Angels Pet Rescue no later than 14 days after the
signing of this agreement, and will receive a refund of my
adoption fee upon receipt by Fallen Angels Pet Rescue of
documentation from the veterinarian as to the dog's state of
health.
