Work permit – issued under the Immigration and Refugee
Protection Act (Canada)
Driver’s licence – (Canadian jurisdiction) valid/non-expired
Certificate of Change of Name – (court order with court seal
for legal name or date of birth change)
Certificate of Citizenship – issued under the Citizenship Act (Canada)
Certificate of Election of Surname
Certificate of Resumption of Surname
Saskatchewan Health card – valid/non-expired
Canadian Indian Status Card – credit card style issued
by the Government of Canada
Canadian Indian Status Card – laminated Polaroid style
Court order – containing the person’s birth date and legal name,
and sealed with the court’s seal
Existing Saskatchewan photo identification
Marriage certificate or certificate of common-law relationship
issued under The Vital Statistics Act (or comparable certificate
from another jurisdiction or church)
Permanent Resident Card – issued under the Immigration
and Refugee Protection Act (Canada) or similar document
issued by the Government of Canada under previous legislation
Signature form for first time drivers between the
ages of 15 and 18/Guarantor form **
Saskatchewan Vital Statistics document – to change existing
information only
Utility bill – (SaskEnergy, SaskPower, SaskTel or Water)
Bank statement or cancelled cheque – with the person’s
home address
Mortgage document
Residential lease
Personal income tax document – with the person’s address
Employment confirmation
Social assistance benefit confirmation
*Documents used to prove residency must include your civic address or land location and be dated within the last 90 days.
**If section A of this form is completed by a parent, it can be used to prove residency and, in this case, a second residency document is not
required. If section C is completed by a parent, it can be used to verify a first time driver’s signature.
Note: Other documents may be accepted by special arrangement.
