breathing pattern Expected Outcome: At the end of 8 hours student nurse-patient interaction, the patient will be able to: a. Establish a normal/effective respiratory pattern as evidenced by absence of cyanosis and other signs/symptoms of hypoxia b. Verbalize awareness of causative factors c. Initiate needed lifestyle changes d. Demonstrate appropriate coping behaviors Interventions 1. Determine presence of factors/physical conditions 2. Assess patients condition 3. Auscultate chest and note rate and depth of respirations, and type of breathing pattern 4. Monitor vital signs, every four hours 5. Monitor patients intake and output 6. Maintain a calm and quiet environment 7. Recommend energy conservation techniques and pacing of activities 8. Encourage adequate rest periods 9. Encourage ambulation or exercise Evaluation
