WEEK 1 [F r o m 1 9 A p r il 2 0 1 0 u n til 2 4 A p r il 2 0 1 0 ] In the early morning of 19 April 2010, trainee report herself at Human Resource Services in KPJ Selangor

Specialist Hospital, Shah Alam at 8.30a.m. The person who is in charge in Human Resource Services of this hospital is Puan Norziha Masran. After trainee report herself at Human Resource Services, Puan Norziha Masran bring trainee to the lab of this hospital. At lab, she introduces Encik Badrul Hisham Abdullah who is laboratory manager and also supervisor of trainee to trainee. After that, Encik Badrul Hisham Abdullah planning the two months schedule for trainee. The schedule included registration, phlebotomy, biochemistry, hematology, Blood Bank, serology, PCR, urine analysis and other. In this week, trainee is doing the part of registration. Trainee learn about what is the passed specimen, which is a specimen which has been check for labeling, correct containers or tube used and condition (clotted, lysed) and posted specimen, which are charged by data entry. About the passed specimen, the lab receives specimen and request form from Accident and Emergency ward (A&E), OPD and wards. The lab also receive patients for phlebotomy and other specimens especially urine. About the posted specimen, the blood samples spinned and allocated to the various sections where the specific test requested are then carried out. The sections are chemistry, hematology, urinalysis, serology, Blood Bank and histopathology. Lab results are either obtained directly from then printer of the analyzer or done manually on result sheet. All the test done are recorded on the section record book and copies of result field. Lab results are sent to the lab office for collection by the representative of the

various wards and OPDs and this is recorded in dispatch book. A&E results however are given priority and once it is ready lab staff has to phone A&E staff to collect the result. From this week, trainee also learns about the procedure for sending the results of blood or other specimen to other institution or private laboratory. The steps are check form or specimen, transfer to external form, prepare guarantee letter, collect or dispatch, receive result. First, medical lab technologies (MLT) checks lab request form, after that, it will transfer to relevant request forms of private labs or institutions with the blood and other specimen. The next step is specimens sent to private labs are collected from KPJ Selangor Specialist Hospital lab by their own countries at fixed time in the morning and evening. After that blood and other specimen sent to institution. Then, contact KPJ Selangor Specialist Hospital Van Service at A&E Department with proper instruction. Before trainee starts do the work in the lab, trainees supervisor reminds trainee that lab is a place that is very dangerous. He reminds trainee that trainee must make sure wearing the proper PPE (Personal Protective Equipment) while trainee was handling the specimen. Trainee also must wear the lab coat and gloves at all times to protect trainees clothing and therefore also protect trainee and also people around from being infected. Trainee reminded by supervisor to use the mask and gloves when trainee was preparing solutions using acids. Trainee also cannot use mouth to suck up any lab solutions or reagents. The expired blood, swabs, sharps, syringe must be discarding into biohazard container and later incinerated by autoclaving or using high heat. All experiments and analyzers must be properly maintained and serviced annually to ensure maximum efficiency. Trainee also doing the filing in this week. In the process doing filing, trainee

knows more about the different type of laboratory request form. The laboratory request form is use to record the results of test blood, urine and so on. Trainee also learn that the blood consist of plasma (55%) and cell component (45%). The plasma part is divided to plasma protein and water. Plasma protein is divided to lipoprotein, fibrinogens, globulin, and albumins. The cell component part is divided to white blood cell, red blood cell, and platelets.
