PURPOSE & ACKNOWLEDGEMENT This is the first time that such a survey has been conducted by flight operations. The aim of the survey is firstly to understand how our flight crew members feel where we are in relation to the companys Saftey First culture as well as our overall Organizational Culture. The Goal of this survey is to therefore; 1. To gauge the flight crews comfort level to be able to achieve open communication with management especially when safety issues are concerned. 2. To gauge what the flight crew understands of the companys Safety First culture. 3. To determine adherence level to new SOP introduction/operational issues. We thank the various departments namely The Regional Director of Flight Operations, The Director Of Flight Operations,Technical & Development, Safety, Flight Operations and Training & Standards Departments for their support and valuable inputs. Last but not least we acknowledge the support of the flight crew that has participated and ensured the success of data collection crucial in this flight operations survey.

SURVEY INFORMATION
The total forms returned were 217 out of the 800 that were sent out to Penang, Kota Kinabalu and Kuala Lumpur bases. This constitutes 27.125% of tabulated replies from the flight crew fratenity encompassing AirAsia Berhad and AirAsia X. The error rate in data capture and replies received were 5 forms out of 110. This is an error pecentage of 4.54%. The tabulation and presentation of data is represented in sub-parts of questions posed in; a) PART A : COMMUNICATION b) PART B : SAFETY CULTURE ( GENERAL ) c) PART B ; SAFETY CULTURE ( COCKPIT ) d) PART C ; UNDERSTANDING & ADAPTATION OF NEW PROCEDURES AND MANUALS Each question posed will be represented by the question,replies as rated from 1 to 5,total forms collected ,percentage in both numerical form. Captured comments are available below these. Further the information is sub-divided into A320 fleet, Commanders and First Officers comments & feedback. Some parts of data received, indicated two replies to the same question and was recorded as two answers for the same question as pilots were viewed to have agreed to both the conditions presented. Feedback Ratings according to in support or opposed to were as follows; 1 = Strongly Disagree, 2 = Somewhat Disagree, 3 = Neither Agree nor Dsiagree, 4 = Somewhat Agree, 5 = Strongly Agree Finally, an overview is provided for quick reference for crew viewing and survey action plan by the respective sub-departments within flight operations.

This question is in supplememnt to the question no:2 which is Unless weather conditions are not conducive, I am usualy stable by? Item (e) reflects 0%, there shouldnt be any reponse to 2.1.
