Child Care Contract/Policy


1. The following agreement is made between Teresa Maisey and __________________ for the care of ____________________________. 2. The child care services will be provided between the hours of _______________and ______________, on the following days: Mon.____ Tues.____ Wed.____ Thurs.____ Fri. ____, or on the following schedule, ______________________________________________________________ You will be charged from the time documented whether you are late or not. The slot you will be filling is a (check one) Part time______ Full time______ 3. If your child is in attendance for less than the full time agreed on, you will still be charged for the time agreed upon. A full contract renewal will be required if your schedule changes. 0-24months 2 & up Daily Rates $23.00 $20.00 (6-9 hours) Hourly Rates $3.50 $3.00

4. See attached Payment Contract 5. There will be a $20.00 charge on any returned checks, and subject to any fees you have cost the provider. If the check has not been made good within 3 days of return, I reserve the right to deny admittance of your child until this matter is resolved. 6. Truancy to pick-up child(ren) is subject to a late fee of: 1st 10 min. free, $1.00 every minute after that. You must have pre-arranged to pick-up at a later time, and the fee for that will be discussed at that time. 7. The parent (or legal guardian) agrees to notify the provider 1 hour in advance for any unscheduled absence. You will still be charged a full-days rate. 8. Holiday closing will be posted 2 weeks in advance. If your child is Full-Time they will be given 4 Holidays per year. 9. Both the provider and the parent are responsible to notify the other 2 weeks in advance on any vacations that will be taken.

10. There is a probation period of 2 weeks in which all children will be assessed for compatibility in this daycare environment. 11. There will be a 2 weeks notice given to the provider if the parent is planning to change daycare providers, or be charged for full day rate for the 2 weeks. 12. The childcare provider is in compliance with federal, state, and local laws. 13. The provider will have the safest possible environment, however accidents do happen. When accidents happen the provider will report every incident and accident to the parent, but is not responsible for it legally or medically. 14. The provider will conduct regular inspections of the facility to ensure it is safe from potential hazards to children. 15. Direct supervision and protection of each child includes: *Sleeping: Children will be checked every 15 min. *Outdoors: Children will be supervised at all times while outdoors. *Off-site Activities: Off-site activities are not available. However, sometimes we go for walks, and in that instant the provider will observe ratios, and also carry all parental contact information in case of emergencies. *Transportation: There is a Plain City Elementary bus stop available for school age children in care. A provider will walk with them while going to the bus stop, and will stay with the children until the bus is there. After school, the children are required to walk as a group to the daycare. If any children are not there, parents will be contacted immediately. 16. The provider will check Utah School Immunization Records (Utah code Law 53A-11-301) at the time of enrollment and again when immunizations come due. 17. The provider will conduct regular inspections of the facility to ensure it is safe from potential hazards to children 18. The provider will supervise the hand washing of the children in care after toilet use and before meals and snacks, and whenever needed. There will be hand washing provided on fieldtrips. The provider will supervise children using the restroom when needed. 19. The childcare provider plans monthly activities that will educate the children in different curriculum activities. 20. Your child(ren) need to come to the providers ready for the day which includes, bathed, dressed, hair combed, and teeth brushed. 21. The provider will clean and sanitize weekly toys and indoor equipment, along with bathtubs if used to bathe children in care, between each use. 22. The provider has completed adult, child & infant First Aid, along with adult, child & infant CPR training and has a First Aide Kit available that includes a First Aid manual. 23. The provider will use a separate crib, bed, or mat for each child who will be present. If one will be needed for your child, the caregiver will notify parent at time of enrollment. 24. The discipline policy in which the provider will use is: Sent to time-out followed by talking with the child(ren) about their actions. Hitting, spanking, yelling, or embarrassing the child is prohibited. Positive reinforcement will be used. The provider will discuss with the parent and try to work on solutions if there is a consistent problem. 25. There will be a sign in & out that will be documented each day the child(ren) is in care. This is how the provider constructs you pay periods by the hours the child is in care.

26. The provider will provide substitutes for when unavailable to care for children for a short period of time, or if an Emergency arises. Immediately... 28. The provider is responsible to report infections or communicable diseases of children in care to parents and county health officials. Such serious diseases include: a. Pretties (whooping cough) b. Rubeola (red measles) c. Rubella (German measles) d. Mumps e. Meningitis (HIB, other) f. Hepatitis g. Tuberculosis h. Gastroenteritis i. Streptococcal (strep throat, scarlet fever, rheumatic fever) j. Food Poisoning k. Influenza l. Cytomegalovirus (MCV) m. Human Immunodeficiency Virus (HIV, AIDS) n. Poliomyelitis o. Infestation (Scabies, head lice) until 24 hr. after treatment begins p. Diphtheria q. Coli 29. The child care provider and/or any other persons on the premise will not smoke, drink alcohol, use of or possession of illegal substances or sexually explicit materials of any kind. 30. Your child is subject to becoming sick if another child is sick or comes down with something contagious. I accept children with minor colds, however I reserve the right to suggest your child be seen by a physician if his/her illness is prolonged and is in danger to the other children in the providers care. The parent (or legal guardian) will be responsible for picking up their children if they show signs of any of these illnesses...... a. a fever of 101 or higher b. illness causing diarrhea or vomiting, 2 or more episodes in a 24 hr. period c. mouth sores (associated with inability to control saliva) d. rash cupped with fever or behavioral change e. infected eyes until after 1st treatment with a medical product f. ring worm until 24hrs after medication has started g. chickenpox until 1 week after onset of rash, or until all lesions have died and crusted over. h. Strep throat until 24 hrs. after antibiotic treatment has started 31. The provider is not responsible for medications to treat sick children. Please provide any medications your child(ren) may need. Tylenol, Motrin, Teething medication, Cough

Syrup, Destine, and at the time of enrollment you will be responsible to bring 1 (one) box of tissues and notified when more are needed. 32. All medicines must have a Medication Release Form completed and signed by the parent in order for the provider to administer any medications. All medication must be in the original container with the original label, followed by a label on it with your childs full name, how much of a dose, the hours between doses. Example (JAMES WHITE 1 tsp. every 4 hrs.) along with some form of spoon or syringe to use for the application of medicine. If the day care provider provides over-the-counter medication to your child, you will be charged .50cents per dose. 33. The following forms are required on the file upon your childs 1st day a. Enrollment form b. Contract agreement c. Health Assessment d. Immunization form (onto a pink state required form) e. Food Program enrollment form f. State assistance paperwork (when applicable) 34. All children will be required to be dressed in appropriate clothing for the weather and season (it is better to have something with your child that doesnt get used, that to not have something they need) including proper attire for playing outdoors. Provide plenty of changes of clothing for a child learning to be potty trained. If clothing becomes soiled- they will be placed in a leak proof container. 35. The parent (or legal guardian) agrees to adhere to the conditions of the attached policy statement. The childcare provider agrees to: a. Provide a safe child care service that supports the physical, social, and emotional needs of the child(ren) in care. b. Provide meals and snacks that are well balanced and nutritional. c. Informs the parent (or legal guardian)..... - In advance if services cannot be provided temporarily due to illness or emergency. - At least two weeks in advance if the services need to be terminated - Of upcoming field trips or other activities outside the home. The parent (or legal guardian) agrees to: a. Pay provider the agreed upon rate at the agreed upon time b. Inform the provider: - in advance if services cannot be provided temporarily due to illness or emergency - one hour in advance if the child will not be in attendance on a scheduled day - at least 2 weeks in advance if the services need to be terminated, or pay the provider for those 2 weeks. - of any change of address, phone number or employment - immediately if any allergy, illness or contagious disease the child may have contracted c. Provide one package of diapers and wipes (when they are gone you will be notified to bring more). d. Provide records of current physicals and up-to-date immunizations for each child. e. Pay the cost to replace (or repair) any equipment or property in excess of $10.00 in value that you or your child(ren) may cause.

f. A phone call from the parent (or legal guardian) stating if someone else will be dropping off or picking up your child.
