N-408 DRUG Generic name: PITOCIN Brand name: OXYTOCIN STOCK DOSE 1ml vial containing 10 units of oxytocin. POSSIBLE ROUTE Intravenous infusion Intramuscular injection INDICATION Used to initiate uterine contractions in a term pregnancy. NURSING RESPONSIBILITIES Prepare IV solution by adding 1ml (10IU) to 1000ml of 0.9 % aqueous sodium chloride or other IV fluid. Monitor frequency, duration, and strength of Contractions Assess maternal pulse and blood pressure, and watch for possible hypertension. Monitor fetal heart rate for signs of fetal distress Monitor intake and output and watch for signs of possible water intoxication, such as headache or vomiting. Do not give oxytocin IV bolus because it may cause rupture of the uterus. It is usually given incorporation.

DRUG Generic name: METHYLERGONOVINE MALEATE Brand name: METHERGINE

STOCK DOSE 1ml per ampoule, containing 0.2 mg methylergonovine maleate

POSSIBLE ROUTE Intramuscular injection Intravenous injection

INDICATION Prevention and treatment of postpartum and post abortion hemorrhage caused by uterine atony or subinvolution.

NURSING RESPONSIBILITIES This drug should be used extremely carefully because of its potent vasoconstrictor action. IV use may induce sudden hypertension and cerebrovascular accident. Give IV slowly over several minutes and monitor blood pressure closely. Check the blood pressure of the client before administering this medication Do not give if the patient has a history of hypertension or an increase in blood pressure.

STOCK DOSE 20mg per ml per ampoule. 10mg per 0.5ml per ampoule

POSSIBLE ROUTE Intramuscular injection Intravenous injection Subcutaneous injection

INDICATION For cervical effacement

NURSING RESPONSIBILITIES Monitor vital signs especially the pulse rate. Monitor the contraction especially frequency, interval and duration. Do not leave the mother because anytime the baby may come out due to the cervix was dilated and effaced. Raise side rails as a precaution because some patient becomes temporarily excited or disoriented and some become drowsy. Advice patient that eyes may be more sensitive to light because buscopan has side effect of photophobia. Advice patient to avoid activities that require alertness until CNS effect of drug are known.

DRUG Generic name: OXYTETRACYCLINE Brand name: TERRAMYCIN

STOCK DOSE 5 mg-10000 units/g

POSSIBLE ROUTE Ophthalmic ointment

INDICATION Prophylactic treatment to prevent opthalmia neonatorum

NURSING RESPONSIBILITIES Instruct the significant others that do not remove the ointment in the newborn eye lid. Wipe both eyes of the newborn using a clean towel, and then apply the terramycin ointment from inner cantus to outer cantus. Apply the ointment at lower eyelid or lower conjuctiva. Oxytetracycline should not be given to premature infants because it may impair skeletal growth or to children under age 8 because it may permanently discolor teeth and cause enemal hypoplasia.

DRUG Generic name: PHYTONADIONE Brand name: AQUAMEPHYTON

STOCK DOSE 0.5 to 1mg within 1 hour after birth, repeated in 6 to 8 hours as prescribed 2mg per ml 10mg per ml

POSSIBLE ROUTE Intravenous injection Intramuscular injection Subcutaneous injection

INDICATION To prevent hemorrhagic disease in neonates

NURSING RESPONSIBILITIES Vitamin K should be administer at the vastus lateralis part of the newborn thigh intramuscularly injection. Phytonadione will not counteract the anticoagulant action Of heparin. Dosage should be kept as low as possible, and prothrombin time should be checked regularly as clinical conditions indicate. This product contains aluminum that may be toxic. Aluminum may reach toxic levels with prolonged parentera l administration if kidney function is impaired. Premature

neonates are particularly at risk because their kidneys are immature, and they required large amounts of calcium and phosphate solutions, which contain aluminum.
