Objectives of this assessment: The objectives of the CHEDs Center of Excellence and Center of Development (COE/COD) Program for Science and Mathematics are to identify, support and develop potential units of excellence in program areas needed for the development of world-class scholarship, nation building and national development (RA 7722, Section 8f). Thus, the project seeks to develop science and mathematics programs in higher education and to increase the number of scientists and mathematicians in academe and industry. This assessment instrument seeks to put together an accurate picture of the unit, which considers the following: a. The mission/vision of the unit; b. Its priorities and strengths; c. Its contributions to the field of specialization and to society; Instructions: 1. 2. This assessment instrument shall be provided beforehand. The unit should provide all the necessary supporting documents. The instrument must be accomplished electronically. It is a locked form in MS Word. Blank form fields for answers are provided. Text fields are configured for unlimited number of characters. Form fields for numbers will only accept numerical answers up to three (3) digits. Three (3) CDs shall be submitted to the CHED. Each CD should contain the electronic file of the accomplished instrument, FIS and the other required attachments. Items in this instrument are cross-referenced to the accompanying Standards for the Selection of COEs and CODs for Science and Mathematics. Specific strengths and objectives should be highlighted. The period covered by this assessment is 1998 to the present. The respondent may attach explanatory notes to any of the items. Those applying as COEs shall answer all of the items. For CODs, the shaded items are optional.
