3. An agreement dated July 1, 2002 covering the term of July 1, 2002 to June 30,
2003. The agreement was for a base amount of $2,000,000.00. The agreement
again was signed by Brian Preski on behalf of John Perzel for the Caucus.
4. An agreement dated September 1, 2003 for the term of 1, 2003, until
June 30, 2004. The agreeluent was fur a bast: amuunt of $1 .The
agreement was signed by Samuel Smith on behalf of the Caucus.
5. An agreement dated July 1, 2004 for the term of July 1, 2004 to June 30, 2005.
The agreement was for a base amount of $1,200,000.00. The agreement was
signed by Samuel Smith on behalf of the Caucus.
6. An addendum to the agreement of July 1, 2004 was signed on June 30, 2005.
This addendum extended the terms of the agreement of July 1, 2004 to June 30,
2006. The terms of payment were dependant upon the following provisions:
GCR and Leader [Majority Leader] agree that new initiatives may
be developed from time to time as subsequently be agreed to by the
parties.
Each month, during the effective period of this agreement, on a
date and a time determined by Leader, the principal agent of GCR
for providing services to the Leader shall personally communicate
with Leader or his designee to report on GCR's activities for the
immediately preceding month. No payment otherwise due shall be

made to OCR in the absence of such reports having been provided.
Any waiver of this report inquiry by Leader for any month or
series thereof shall not be construed as a waiver of any subsequent
months reporting requirement.
Under the terms of the agreement, the "Leader" was defined as Samuel H.
Smith, Majority Leader of the Pennsylvania House of .Representatives.
This addendum was signed by Samuei Smith' for the Caucus.. (Over
$1,200,000.00 was paid during the term of this addendum.)
7. A second addendum signed June 30, 2006 for the term of June 30, 2 0 0 ~ until
November 30, 2006, extending all the terms of the July 1, 2004 and the first
addendum of June 30, 2005. This addendum again was signed by Samuel Smith
on behalf of the Caucus. (In excess of $600,000.00 was paid to OCR during the
term of this addendum).
8. Agreement dated May 1, 2007 for the term of May 1, 2007 to November 30,
2008. The cost of the agreement is a base of $34,877.92 a month. This
agreement was signed by Samuel Smith on behalf of the Caucus. (This
agreement was terminated in August of 2008)
As previously stated, the Caucus paid in excess of $9,286,000.00 under the above listed
agreements.
OCR also had two contracts with the House Republican Campaign Committee. The first
contract was dated May 1, 2003 and was for the term of April 1, 2003 to December 31, 2004.
