RATIONALE

This study aims to provide different information regarding the health status of the

Sto Niño community especially the people living in the Purok V-A and V-B. This study

also aims to disseminate information about the family structure, characteristics and

dynamics, socio-economic and cultural characteristics, home and environment, health

status of each member, and lastly the values and practices on health promotion or

maintenance and disease prevention.

The information gathered during the zonal survey will assist the community to

identify problems affecting the health of the people. These appropriate inventions can be

formulated to address the problems.


LIMITATION OF THE STUDY

The ASTHREA gathered information take from zonal survey thru interview and

observation in zone V-A and V-B of Barangay Sto Niño with the assistance of the Public

Health Midwife and Barangay Health Worker assigned in the area.

The information gathered by ASTHREA group will be more comprehensive,

meaningful and far-reaching if it covers other area of the purok V-A and purok V-B.

Unfortunately, the group was not able to gather all data of the community since some

families are working, thus they were not included during the house to house survey.

Others did not want to participate due to some personal reasons.


TOOL USED

The tools used in conduct of the zonal survey are survey questionnaires, interview

guide and observation checklist. The community survey was done by checking filling up

the instrument and was subjected for completeness, accuracy and reliability of the

information collected. The information was consolidated after the conduct of the zonal

survey. To facilitate data collation, the group developed categories and classification of

responses to make sure that the categories are mutually exclusive, choices do not overlap

and exhaustive to anticipate to all possible answer that a respondent may give. Finally,

data were summarized through manual tallying of the data which involves entering the

responses into prepared tally sheets all responses.

of this zonal survey, to include the following people:

conducting house to house survey.

INTRODUCTION

The zonal survey was conducted by the ASTHREA group to obtain and gathered

important data to serve us baseline information for community health care. Likewise,

priority nursing interventions derived from, the information gathered during the zonal

survey. Thus, the zonal survey is an affective tool to identify the community health

problems and come up with appropriate interventions to address the problems.

The study was conducted at [purok V-A and purok V-B. It specifically attempts to

answer the following questions:

1. What is the population of purok V-A and V-B.?

2. What are leading causes of morbidity and mortality?

3. What are the resources available in the community for health care?

4. What are the socio-economic, environmental, cultural and political factors in the

community?
STATEMENT OF OBJECTIVES

At the end of the community zonal survey, the ASTHREA group will be able to:

1. Establish intrapersonal and interpersonal relationship with the clients or families in

the community.

2. Gather and obtain information through the use of community survey tool, interview

and observation.

3. Identify the presence of community resources for health care.

4. Recognize the trends and patterns in terms of health needs and problem of the

community.

5. Summarize systematically all the information gathered in the community.

6. Identify the feedback mechanism on health activities and projects of the community.

7. Determine the health attitudes and behavior of community by actively involvement

and participation in community health projects and activities.

8. Demonstrate active involvement in conducting house to house survey, interview and

observation.

9. Coordinate with appropriate offices in the community such as the Sangguniang

Barangay and Barangay Health Center.

10. Provide the community with the update information gathered during the zonal survey.
