WAIVER AND RELEASE FORM

THIS FORM MUST BE READ CAREFULLY AND SIGNED BEFORE THE PARTICIPANT IS PERMITTED TO TAKE PART IN THE CANON PHOTO MARATHON 2011 ON BY SIGNING THIS AGREEMENT, THE PARTICIPANT AFFIRMS HAVING READ IT. .

Warning and Acknowledgement of Risk and Damages I have entered the Canon Photo Marathon out of my own free will. I acknowledge that I am in good physical condition and have no medical problems that would affect my ability to participate in the event. I voluntarily agree to assume the full risk of any injuries, damages or losses of properties, regardless of severity. Should I suffer an injury or illness, I authorize officials of the attending emergency services to use their discretion to have me medically treated and transported to a medical facility.

I willingly agree to comply with the stated terms and conditions for participation. If, however, I committed damages to any personal property during the event, I agree to be accountable and hold myself responsible for my actions and any other additional cost brought by the damage.

Liability Release I acknowledge that I have read and understood the above warning and acknowledgement of risk of injuries, damages or losses of properties. I, for myself, and on behalf of my heirs, personal representatives and next of kin, hereby release, hold harmless and promise not to sue Canon Marketing Philippines, Inc. its sponsoring organizations, their offices, volunteer, staff and agencies, with respect to any and all injuries, damages and losses that may arise from my participation in this event.

Material Consent I agree that Canon Marketing Philippines, Inc. will have the right to reproduce, exhibit or publish the photos I submitted or photos taken of me during the event. Canon Marketing Philippines, Inc. may display the prints in a public venue or publish the prints for the purpose of promotion without financial compensation to the photographers; however the copyright of the photograph still belongs to the photographer.
