Customer Profile

Providence Health & Services of Oregon is a not-for-profit network of hospitals, health plans, physicians, clinics, home health services and affiliated health services. As part of the greater Providence Health & Services, with services in Alaska, Washington, Oregon, Montana and California, they continue the tradition of caring that the Sisters of Providence began more than 150 years ago.

The Challenge

The Portland Medical Center desired insight into unnecessary extended stays in the hospitals Intensive Care Unit (ICU), as well as determining which sets of surgical procedures and medications, and at what stages, seemed to have the greatest impact into improving the wellness of their patients. These and other major concerns were very difficult to assess because even a single analysis required manually gathering information from multiple hospital applications, sorting through that data, performing manual spreadsheet calculations, and determining some correlation of that data with known benchmarks. This type of methodology was proving to be very time- consuming, not providing adequate information for trend analysis, and too late to perform any actions on existing patients in the ICU. The major goal for the ICU Dashboard Project was to provide visibility of key business and clinical information to executives, administrators and physicians that are actionable and provides opportunities to improve quality of care, while highlighting opportunities for cost reduction and containment.

Summary
Mantis worked closely with both clinical and IT staff at Providence to understand the business and technical challenges associated with this project. Using an Agile methodology, our team was able to deliver the right solution based on the SAP Business Objects platform to meet everyones expectations and make a difference at Providence in the ICU.

The Solution

Using an Agile methodology, Mantis designed and built an end-to-end patient information and diagnostics data warehouse for the Intensive Care Unit to support their current and future needs. The design provided flexible functionality that could be incorporated by other hospitals in the Providence Health Care system. Applications were transformed via Extract, Transform, and Load (ETL) process into a newly designed dimensional data model using SAP Business Objects Data Integrator and Microsoft SQL Server 2005 database. Using Data Integrator, Mantis was able to create an automated and scheduled system to extract data on a daily basis from source systems, analyze and transform the data, and load it into the dimensional model for processing by the analytics, dashboards and reports. The ETL process also utilized complex workflow logic and custom algorithms to transform raw medical measurement input data into Benchmark level outputs, including APACHE IV, Severity of Illness, and Sepsis Levels. This would enable the clinical teams to provide the best treatment for the illness, while enabling the business to answer questions regarding the cost effectiveness, and ultimately yielding better decisions regarding patient care overall.

High Fidelity Wireframes


The Mantis team designed and built a semantic layer, utilizing the Business Objects Universe, onto the SAP Business Objects Enterprise platform to support a more rapid development, uniformity, and simplicity of dashboards, pre-made reports, and ad-hoc reports. Business Objects Query as a Web Service technology was implemented to support dynamic data feeds to the dashboards and reports.

Key Performance Indicators (KPIs) were visualized on business and clinical dashboards using SAP Business Objects Xcelsius. Each metric was based on a fact measure from the data model, and each dimension was abstracted to provide on- demand filtering and drill-down capabilities for end users. Dynamic filtering types included begin and end dates, severity of illness, types of illnesses, diagnostic codes, medical procedure codes, and others depending on the module selected. In addition, the data model enabled drill-through from the summarized KPIs on the dashboards into the atomic, transaction detailed data for those needing to get at this level of information. This drill-down information was presented using linked Crystal Reports with passed parameters from the dashboard. Linked detail reports contained key patient and visit information, such as length of stay, age, benchmark category levels, surgery procedures performed, medications, mortality, diagnostic codes and procedures, and hospital costs. Granularity of data could be viewed at multiple levels such as by visit, by day, by shift, and by hour.

We just completed the demo of the dashboard and it went really well. We had more than 70 people in the room including administrators, finance people and physicians and they all wanted to know when and how can I have something like that?

Results and Benefits

In conjunction with formal training conducted by SAP/Business Objects to key Providence IT staff, our development team provided additional training to our customer with the usage of these Business Objects designer applications. Our customer could then service themselves with future changes in business requirements as well as design, create, and implement new functionalities in their new system. The Medical Director for Providence Portland Medical Center demonstrated the working capabilities of the ICU Dashboard to a wide audience of Providence Health and Services members, and gained high levels of excitement. By having the power of information now available to them, the Providence Staff anticipates significant cost savings and improved urgent care to both the hospital and their highly critical patients. Based on feedback from other staff members and directors, they also anticipate future adoption of the new system by other hospital facilities of Providence.
