REQUIRED MATERIAL
1 Professional autobiography: Tell us about your career, being sure to discuss two things: 1) how and why you moved from one stage to the next and 2) what difference your work has made to the world. Not to exceed 750 words. (6 copies)

Professional goals: Tell us what you would do during a fellowship year and how that would fit in with your professional goals. Not to exceed 500 words. (6 copies)

If you are a freelance, give us a list of all your jobs in the last 12 monthswhat you did, for whom and, if your client is not widely known, give us some idea of who or what it is. A few words for each job is sufficient. (6 copies)

Samples of professional work: (6 copies each of 6 samples) Select works that best show your interests and abilities. All samples should have been published or broadcast within the last 24 months. Do not submit any item that is irreplaceable. (Materials will not be returned.) If original work is not in English, please provide an English translation. Written work: All clippings should be copied onto 8 1/2 X 11 paper. Please try to avoid reducing type size. Instead of whole books, submit one chapter. With newspaper series, send only the first and last articles, with a brief outline of other items in the series. Editors should indicate their involvement (assignment, copy-editing, picture selection, picture use, layout, etc.). To be submitted on NTSC formatted video cassette or audio cassette (6 copies) with a total running time of up to 30 minutes and a typed manuscript or synopsis (6 copies) indicating where each item (program, story or segment) begins on the tape. Directors or producers should indicate their involvement (assigning, editing, directing, producing). Submit work samples as printouts on paper, and, if possible, provide the URLs. Submit photos or artworks either as published clippings, or, if you prefer, make new copies for us.
