Curvy QT Model Search 2012 Curvy QT is bringing in the New Year with a model search!

Curvy QT believes sexy is a vibe, and not a size. Thats why we strive to showcase our clothing on models that dont necessarily fit the industry standard. Winner will receive: A Curvy QT Photo shoot complete with makeup by Miss Bellisima A Photo shoot and lunch with Rosie Mercado $250 in Clothing A Make-Up Gift Bag from Miss Bellisima Prominent placing on www.curvyqt.com Requirements: Ladies ages 18 and up may complete an entry form and submit with $20 entry fee. A portion of the entry will be donated to the Susan G Koman Foundation. Contest is open only to legal residents of the 50 United States. Void in Puerto Rico, other United States territories, outside the 50 United States, and where prohibited or restricted by law. Winner will be responsible for transportation to the photoshoot, unless otherwise updated by the sponsor. Entrants must be size 14 and up. Entrants must submit one (1) 4 x 6 or larger headshot photograph and one (1) 4 x 6 or larger full body shot. Photographs must not contain lingerie, nudity, or obscenity as determined by Sponsor in its sole discretion. Photographs must be taken within three (3) months prior to entry date. All entrants must not be listed or represented by a talent or model agency or Management, while involved in the model search.

WINNER SELECTION:
Entries will be submitted to Curvy QT either in person at announced Curvy QT events, or by mail to Curvy QT Model Search, C/O Y. Orozco, PO Box 3081, Santa Ana, CA 92703. Submissions must be accompanied with a check or money order payable to Curvy QT and postmarked no later than February 28th. Submissions may also be via email to CurvyQT_Model_Search@curvyqt.com or fax to 1-714-436-1168. Entrants may also pay entry fee at www.curvyqt.com. Entrants will be notified via email when complete application and entry fee have been received. Start date is December 22, 2012 and ends on February 28th, 2012. A committee of judges selected by Sponsor will choose twenty (20) semi-finalists on or about March 6, 2012. All decisions of the judges are final in all matters relating to this Contest. The semifinalists will be notified by email or telephone by March 11, 2012. The twenty (20) semi-finalists photos will be posted on Curvy QT Facebook Fanpage at https://www.facebook.com/pages/Curvy-QT/245753652138260 for public voting beginning at 12:00:01 am (PST) March 16, 2012 through 11:59:59 pm (PST) March 30, 2012. Public and entrant voting shall be limited to one vote per entrant per day. Voters must like the Curvy QT Facebook Fanpage in order for vote to count. 1

5 Finalists will be announced on March 31, 2012. Judges will select winner from the five Finalists, and announce the Curvy QT grand prize winner on April 5th, 2012. Winner will be required to execute and return an Affidavit of Eligibility/Release of Liability/Publicity Release within three (3) days of issuance or an alternate winner will be chosen. By entering and/or accepting the prize, Winner consents (except where prohibited) to the reasonable use by Sponsor of the Winner's photograph, voice, likeness and/or image, contents of the entry form and the name and age of the Winner without any further compensation for any future promotional activity related to this Contest.

General:
By entering this Contest, entrants agree that Sponsor shall not be liable for personal injuries, death, damages, expenses or costs or losses of any kind resulting from participation or inability to participate in this Contest or acceptance of or use of or inability to use the prize or parts thereof, including, without limitation, claims, suits, injuries, losses and damages related to personal injuries, death, damage to or destruction of property, rights of publicity or privacy, defamation or portrayal in a false light (whether intentional or unintentional), whether under a theory of contract, tort (including negligence), warranty or other theory. Winner releases Sponsor from any and all liability and responsibility with respect to the prize (including any property loss, damage, personal injury or death). Winner acknowledges that the Sponsor has neither made nor is in any manner responsible for any warranty, representation or guarantee, expressed or implied, in fact or in law, related to the prize.

LIMITATIONS ON LIABILITY:
Sponsor will not be liable to Winner or any other person for failure to supply any prize or any part thereof, by reason of any acts of God, any action(s), regulation(s), order(s), or request(s) by any governmental or quasigovernmental entity (whether or not the action(s), regulation(s), order(s) or request(s) prove(s) to be invalid), equipment failure, terrorist acts, earthquake, war, fire, flood, explosion, unusually severe weather, hurricane, embargo, labor dispute or strike (whether legal or illegal), labor or material shortage, transportation interruption of any kind, work slowdown, civil disturbance, insurrection, riot, or any other cause beyond Sponsor' control.
CAUTION: ANY ATTEMPT BY A CONTESTANT TO DELIBERATELY UNDERMINE THE LEGITIMATE OPERATION OF THE CONTEST IS A VIOLATION OF CRIMINAL AND/OR CIVIL LAWS AND SHOULD SUCH AN ATTEMPT BE MADE, SPONSOR RESERVES THE RIGHT TO SEEK DAMAGES AND OTHER REMEDIES FROM ANY SUCH PERSON TO THE FULLEST EXTENT PERMITTED BY LAW.
