1. Candidates should apply only through 'ON LINE' through the website http://www .keralaesic.com, www.kl.esicexams.org, www.lasagu.com .

No other means/mode of application will be accepted. Online Registration starts on :21.12.2011 10.00 A.M Online Registration closes o n :16.01.2012 5.00 P.M 2. Candidates should have a valid E-mail ID. In case a candidate does not have a valid E-mail ID, he/she should create his/her new E-mail ID before applying on line. Before filli ng up the ON Line Application the candidate has to click the link - 'I have read the instruction'- at the end of this instruction and you will go to the site where you will be advised to enter your E mail ID. The ID will be confirmed and you will have to obtain your verification Code from your E mail Account and enter this code to proceed. 3. Candidates are advised that before filling up - on line application form - th ey must get a Demand Draft [ Cheque/Postal order will not be accepted ] for Rs.225/- (Rupees Two hundred a nd fifty only) for Upper Division Clerks and Rs.125/- [Rupees one hundred and twenty five only] for Multi Tasking Staff obtained from State Bank of India or its subsidiary banks or any Nationalised Bank on or after the publication of the Newspaper advertisement in favour of ESI Corporation and payable at TRICHUR. Without details of the Demand Draft, the system will not accept the application. However, Candidates belonging to SC, PH, EXSM, Female applicants an d Employees of ESIC are exempted from payment of fee. Candidates are advised to keep particulars of educ ational qualifications, work experience details, caste certificate details, and other personal details ready as these details are required to be entered in the on-line application. 4. There are different categories i.e. General, SC, OBC and horizontal reservati on will be available for Ex- Servicemen and physically challenged as per details available under General Instructions. The candidate will be considered only for the category for which he/she has fill ed up the form subject to fulfilling of prescribed eligibility conditions. For example, if a candidate has filled up under General Category though he/she belongs to SC/OBC/Ex- Serviceman Category, he/she will be considered only for Ge neral category (provided he/she fulfills eligibility conditions as a General category Candidate ) irrespective of his belonging to the SC/OBC/Ex serviceman category. Similarly, if a General Candidate has fill ed up SC/OBC/Ex-Serviceman form, his/her candidature will stand cancelled being not SC/OBC/Ex-Serviceman. C andidates are, thus, advised to fill up the form for the category they belong to 5. Candidates who are employed in Govt. /Semi Govt. /Autonomous bodies etc. shou ld send hard copy of their applications through proper channel. However they may send an advance copy of th eir application along with demand draft (if payable) and other documents [if applicable] so as to reach thi

s office on or before the last date. 6. Fill in the required details in the ON LINE Application therein including DD number etc. and click SUBMIT button at the end of the ONLINE application form. A Unique User Name and Passwor d will be generated and sent to your E mail Account. The candidates should take note of the User Name for the pu rpose of writing at the back of DD. Candidates, in their own interest, should note down this Computer Generated User name and password carefully and retain the same for re-login, downloading the admit card and for further referen ces. 7. After applying on-line take a print out of System generated ON LINE APPLICATI ON FORM in A-4 size paper ( You will receive the User Name and Password through your e-mail. Login with th ese details to take the print out. ) [a] Candidates applying for Upper Division Clerks, should submit system generate d hard copy of online application duly signed by the candidate at the appropriate space/s provided wit h attested photograph, by a Gazetted Officer, pasted on it and Demand Draft [as the case may be] No other d ocument are to be attached at this stage. However the copies of the application and testimonials will have to be submitted by the successful candidates in the Part I examination at the time of calling for Computer Skill T est.
