PASSPORT APPOINTMENT REQUEST FORM - OUTREACH 2012 Only a limited number of Passport Applications can be processed during the outreach this year, as we launch the mobile ePassport service. To facilitate a smooth flow of applicants, an appointment system has been imposed requiring passport applicants to secure an appointment/slot prior to the outreach. This will allow the Consulate General to pre-process the applications and reduce the waiting-time for applicants during the outreach itself, as we will be able to advise applicants with an indicative time-frame for their data-capture process. Only applicants whose documents have been received by the Consulate General at least 7-days prior to the scheduled outreach can be accommodated. Those who are unable to secure an appointment are invited to come to the Consulate General during our regular office hours, 9am to 4pm, Mondays through Fridays except during official holidays. Fully Accomplished Passport Application Form (no photo for straight renewal of the passport, without amendments) Xerox copy of the current passport showing the data page, the page with the latest stamps of entry to the US, the back page showing the signature of the Consular Officer Xerox copy of the proof of current Philippine Citizenship US visa, green card, INS notice of action, etc Using married name for the first time in the Passport Xerox copy of the signed Report of Marriage/ Marriage Certificate issued by the NSO/ or application for Report of Marriage (see http://www.pcgny.net/2009/legalReportOfMarriage.html) First-time Passport applicant must submit signed Report of Birth / Birth Certificate issued by the NSO/ application for Report of Birth by mail (see http://www.pcgny.net/2009/legalReportOfBirth.html) Payment in the form of money order addressed to Philippine Consulate General New York: First time / straight renewal $60.00 (additional fees apply for Lost / Irregular Passports)

Confirmation email / appointment schedule original passport original proof of current Philippine citizenship (green card, INS notice, US visa, etc) for those using the married name for the first time, original signed Report of Marriage / original MC-NSO for those applying for a Passport for the first time, original signed Report of Birth / original MC-NSO Self-addressed, pre-paid envelope (USPS priority mail w/ delivery confirmation recommended) to receive your new passport by mail (1 envelope per passport application)
