The Consular Outreach Team will be at the site from 10am to 3pm to process PASSPORTS, DUAL CITIZENSHIP, LEGALIZATION of documents and OVERSEAS ABSENTEE VOTER (OAV) REGISTRATION for the 2013 Elections.
DOCUMENTS TO BRING PERSONAL APPEARANCE REQUIRED FOR ALL PASSPORT APPLICATIONS WITHOUT EXCEPTION Straight Passport Renewal: 1) Accomplished application form; 2) Proof of identity current Passport, original and photocopies of the data-page, page w/ entry stamps & last page; 3) Proof of current Philippine citizenship original and photocopy of US visa, I-94, Permanent Resident Card, INS notice, etc.; 4) FeeMoney Order $60.00. The new ePassport will be available after eight (8) weeks. Those who have applied for renewal are also eligible to apply for Passport Extension for an additional fee of $20.00 (released on the same day) For Lost Passport, Amendments, etc: Visit www.pcgny.net for additional requirements. The ePassport Data Capture system is only able to handle a limited number of applicants. PASSPORT APPOINTMENT REQUEST FORM - OUTREACH 2012 Only a limited number of Passport Applications can be processed during the outreach this year, as we launch the mobile ePassport service. To facilitate a smooth flow of applicants, an appointment system has been imposed requiring passport applicants to secure an appointment/slot prior to the outreach. This will allow the Consulate General to pre-process the applications and reduce the waiting-time for applicants during the outreach itself, as we will be able to advise applicants with an indicative time-frame for their data-capture process. Only applicants whose documents have been received by the Consulate General at least 7-days prior to the scheduled outreach can be accommodated. Those who are unable to secure an appointment are invited to come to the Consulate General during our regular office hours, 9am to 4pm, Mondays through Fridays except during official holidays.
