Introduction This template is provided by DNV in order to ensure uniform reporting of Ultrasonic Thickness Measurements (UTM) for ships.

It is to be used whenever such data are being reported to DNV. Overall Objective. DNV uses UTM data for the following main purposes: 1. Identification of areas of substantial corrosion and thicknesses below the allowable minimum. 2. Verification of the ship's longitudinal strength. 3. For Nauticus Operation ships: Statistical analysis of diminution and import of data into a 3D model of the ship. 4. For CAP (Condition Assessment Program): Statistical analysis of diminution. For all of the above purposes DNV relies on high quality data and reporting from the UTM provider in order to achieve it's objective of safeguarding life, property and the environment. It should be noted that items 3 and 4 above require higher precision and data consistency than for a traditional paper report. General The report is to be prepared onboard during measurements, and a digital copy of a preliminary UTM report shall be given to the attending DNV surveyor at completion of the measurements, and as required during the measurements. A kick-off meeting with the attending DNV surveyor is to be held prior to commencement of measurements. The structure and organisation All required information must be filled in under the relevant sheet. of the data are not to be altered. A paper report, signed by the attending surveyor is required in addition to the electronic report. The sheets; Deck, Side, Bottom, Inner bottom, Inner deck, Longitudinal bulkhead and Transverse bulkhead represent watertight boundaries, all other data are to be organised under the "Internal structures" sheet. Hatches, etc. may also be reported under the "Internal structures" sheet. The boundary data are to include plating measurements and stiffener measurements. Internal structure data are to comprise all other internal structure like stringers, webframes, floors, girders, swash bulkheads, etc. Complete transverse sections are to be reported under the "Transverse sections" sheet. Transverse sections shall have the location ( frame number ) clearly indicated, and have both stiffeners and plates included. For CAP surveys only: The data reported under Transverse sections are also to be included under the respective sheets for Deck, Side, etc. Number and location of measurement points For Class surveys reference is made to DNV Rules Pt.7 Ch.1 and Instructions to Surveyors I-C5.1 Ch.1.5

For CAP surveys reference is made to "DNV CAP Thickness Measurement Specification". All documents are available from Transverse sections Transverse section measurements include both the web and the flange of all longitudinal stiffeners and the plate thickness in every spacing between every longitudinal in the deck and bottom area, as well as one thickness measurement of every plate in the ship's side and longitudinal bulkheads between the deck and bottom area. By "deck and bottom area" is meant the upper and lower 15% of the ship's transverse section. Bulk carriers - Sideshell frames For bulk carriers the IACS Unified Requirements UR S31 may apply. For the sideshell frames of these vessels it is required to record values for T-coat. (Below the thickness of T-coat measures like coating and/or reinforcement of the structure have to be implemented). Location according to zones A, B, C or D is also to be recorded. Ref. IACS UR S31, available from www.dnv.com Area reduction table DNV's list of minimum allowable thicknesses (See Class min thk. below) is based on the assumption that the area reduction in deck and bottom is within a specified limit (normally 10%). Verification of this may be necessary. Instructions and calculation templates for this as well as other supporting documents are available from DNV at www.dnv.com Data columns Tank / Space Required Give a unique tank identification for the tank / space in which the measurement was taken. Use consistent tank codes like DB2 for Double bottom No. 2. Do not mix codes like DB2, DB 2, DB-2 etc. as these will be interpreted as separate tanks. For connected tanks, like double side / double bottom tanks, use codes that indicate the part of the tank. Ex: WB2-DS and WB2-DB. Do not include port/stbd in the tank code for symmetrical tanks. Tank / Space on reverse Required for plate measurements only Give a unique tank identification for the tank / space on the reverse side of the plate on which the measurement was taken. Include this for plate measurements only - not for stiffeners. Structural Element For Information Give a description of the structural element in every row. Element / Frame No. Give an element or frame number in every row. For Information

Required for Draw ref. Verification A unique reference to a point on attached sketches must be given for each measurement point in the UTM report.

It is proposed to use incremental point references 1, 2, 3, . etc. in combination with the sketch title or code. Ex: S4-13 (Sketch 4, Point 13) Sketches may be included in this report under the "Sketches" sheet, or attached in a recognisable graphical file format. (.dwg, .gif, or similar) Orig thk. Insert the original plate thickness. Required

Class min thk. Required Insert DNV's Class minimum allowable thickness. (Not the CAP 2 limit, or any other limit) A Ship Specific list of minimum allowable thicknesses for various structural elements is available from DNV (mtpno864@dnv.com) for all ships >100m. length. For ships < 100m. length, a list will be prepared by the surveyor ( in general allowing 20% thickness reduction for plates, 25% for the web and flange of stiffeners as well as transverse bulkhead plating, and 20% for the web and 25% for the flange of web frames. It is also normally available in the vessel's ESP (Enhanced Survey Program) file onboard. Gauged port / starboard Insert the gauged thickness. Required

Calculated (write Diminution protected) The diminution in mm and % is calculated automatically. Renewed port / starboard Required if applicable (CAP and NAUTICUS only) Insert the plate thickness of the new plate if the representative area has been renewed. Note that all the area the point represents must be renewed, not only a local patch. If the point grid is wide in the area in question, the number of points must be increased, or the full area renewed. Unless these requirements are fulfilled: Do not update the "Renewed" column. Please observe the requirements to unique references to sketches as described under "Draw ref." above. Updating of UTM data after renewals is particularly important in connection with items 3 and 4 under "Overall Objectives" above. Comments, Defects found For Information Insert general comments and information considered relevant for the structural integrity of the vessel. E.g. structural defects found, such as cracks, buckling, indents or pitting. Required if data are to be imported into a 3D X1, Y1, Z1 (Nauticus only columns) NAUTICUS model X1, Y1, z1 are the global coordinates of the measurement points. x1 = longitudinal position, origin in AP and positive forward format #44+500 (mm) y1 = transverse position, origin in centreline, positive on starboard side +5000, -2900 (mm) z1 = vertical position, origin in baseline, positive upwards +12000 (mm) Example #44+300, -2000, +3000
