1 This form, once completed and returned to Essar (including requested attachments), forms the basis of a desktop commercial and technical pre-qualification evaluation of your company. 2 The whole of the contents of this questionnaire will be treated confidentially and will not be distributed except on a need to know basis. 3 Except where specified, complete the questionnaire in full and do not answer any of the questions by reference to other documents supplied by your company. 4 Please return the completed questionnaire, along with any attachments to vendordevelopment@essar.com 5 All questions shall be answered in English. If a question is not applicable, please state as such. If documents, to be supplied in context of this questionnaire, are not stated in English, a copy of these documents translated in English shall be sent together with the original document. 6 The qualification process consists of two parts: Completion of this questionnaire bythirdvendorauditor verifying the vendors Assessment by Essar or designated the party capabilities. 7 Suppliers are requested to fill in the required information in the highlighted cells only.
