    FifthAnnualLoveWalk/Run WaiverandReleaseForm InconsiderationfortheopportunitytoparticipateintheFifthAnnualPacificPalisadesLoveWalk/Run (theEvent),Iagreeasfollowsformyself,andforthechild(ren)thatis/areundermysupervisionwho participateand/orattendwithme: 1.Iherebywaiveandrelease,formyself,thechild(ren)thatis/areundermysupervision,myheirs, executorsandadministrators,anyandallrights,claims,liabilitiesandcausesofactionwhatsoeverIor thechild(ren)thatis/areundermysupervisionmayhaveagainstthePacificPalisadesWomensCluband theDr.SusanLoveResearchFoundation,theiraffiliatesandtheEventoperatorsandsponsorsandeach oftheirrespectiveofficers,directors,employeesandagents(theEventParties)relatingtoorarising frommyorthechild(ren)thatis/areundermysupervisionparticipationintheEvent,includingbutnot limitedtopersonalinjury. 2.IrecognizetheEventonSunday,May20,2012hasinherentriskofinjuryandIherebyassumethat risk,andonbehalfofmeandthechild(ren)thatis/areundermysupervision.

    on.IfI,thechild(ren)that is/areundermysupervision,orpet(animal)causesinjurytoanypersonordamagetoanyproperty whileparticipatingintheEvent,IherebyindemnifyandholdharmlesstheEventPartiesfromand againstanyandallclaims,suits,actions,losses,damages,andexpensesrelatedtoorarisingfromsuch injuryordamage. 3.IunderstandthatIamsolelyresponsibleforthehealthandsafetyofmyselfandthechildrenthatare undermysupervisionattheEvent.IamcertainthatIandthechildrenthatareundermysupervision arephysicallycapableofparticipatingwiththeEvent.Iunderstandthatanyphysicalactivityincluding thephysicalactivityinvolvedwithmyparticipationwiththeEventhasinherentriskofinjuryandI assumethatriskonbehalfofmeandthechildrenthatareundermysupervisionduringtheEvent. 4.IherebygivemyconsenttothePacificPalisadesWomensClubandtheDr.SusanLoveResearch Foundationandtheiraffiliatestousemyandthechild(ren)thatis/areundermysupervisionname(s) andphotographs,videoandfilm(photos)ofmeand/orthechild(ren)thatis/areundermysupervision takenbefore,duringoraftertheEventinadvertisingandpromotionalmaterialsforthePacificPalisades WomensClubandtheDr.SusanLoveResearchFoundation,includingbutnotlimitedtotheinternet,

    withoutcompensation.Iagreethatnoadvertisingorothermaterialsneedbesubmittedtomeorthe child(ren)thatis/areundermysupervisionforapproval.IagreethatallPhotosofmeand/orthe child(ren)thatis/areundermysupervisionusedbythePacificPalisadesWomensClubandtheDr. SusanLoveResearchFoundationandtheiraffiliatesareownedbythePacificPalisadesWomensClub andtheDr.SusanLoveResearchFoundationandmaycopyrightmaterialcontainingthesame.Ihereby release,discharge,andagreetosaveharmlesstheEventPartiesfromanyliability,including,without limitation,anyclaimsforlibelorinvasionofpublicity/privacy,byvirtueofanyuseofmyorthechild(ren) thatis/areundermysupervisionnameand/orPhotos,including,anyalterationofsuchPhotos,whether intentionalorotherwise. IhavereadandunderstandtheWaiverandRelease,anddeclareallinformationistruthfulandaccurate.
