1997 - Mum and Dad separated after 20 years together. This has had a detrimental effect on me and my siblings.

1999 - Met Christopher Vink. 2001 - Became engaged to Chris and we bought house together which I am currently living in. Later in the year Chris and I separated. 2002 - In March I saw an advertisement in the local paper which Kevin had inserted. I then replied to this and thats how I met with Kevin. 2002 3rd June - I found out Chris had died. He was only 27 years old. I was very upset. His funeral was on the 7th of June. That night Kevin proposed to me. I felt that was very insensitive of him to do so and very disturbing for me. 2002 September - I found out I was pregnant with Kevins child Shay ********************************************************** 2003, 16th April - At 8.56am, Shay come into the world. Kevin was very controlling and didnt allow me to bond with Shay. He was insisting he does everything that a mother should be doing with her child. That made me insignificant. I wanted so much to hold Shay but he wouldnt let me. He just took over my baby and my life. 2003 July - Kevin ended our relationship after an argument. Later I said words to Kevin that I deeply regretted and I had no intention of following through. I regret to this day expressing suicidal thoughts. I was at the time hoping Kevin wouldnt leave me. Kevin got police involved, and I was taken to Dandenong Emergency Department. ***Refer to notes DHS took Shay away from me for a few weeks and Shay went to Kevin. I was allowed to see Shay with my mother going and getting Shay. 2003 12th August - I was voluntarily admitted to the Monash Mother Baby Unit at Clayton for two weeks. In that time Kevin would come in to see Shay in the frustration of seeing him made me angry. After two weeks I was discharged due to an outburst with Kevin. Shay was then put in Kevins care on Interim accommodation order till 17th September. Page 1

I then returned to the Mother Baby Unit in Clayton for 9 weeks with Shay. Kevin would come in everyday which would greatly disturb me. 2003 **16th September I saw Dr Fred Stamp Psychiatrist at Delmont Private Hospital. This was organised by Valention Gonsalves, a DHS worker. See Report 22nd September I saw Dr Fred Stamp Psychiatrist 2003 11th November - Another appointment with Dr Fred Stamp Consultation Psychiatrist see report **************************************************************** I finished contact with Kevin ???????2004, 17th December - Court orders made at Family Court for Shay to live with me and Kevin have access. REFER TO COURT ORDERS **2005 - Kevin and I tried our relationship again, and only lasted three months **************************************************************** 2006 22nd Feb- Kevin rang cops and I was put in Emergency again. He and his mother turned up to take Shay but he wasnt allowed. Shay went home with a friend who was approved by DHS. 2006 23rd Feb We tried once again to reconcile, which was to be the last time as it ended up with Kevin assaulting me because he wouldn't let me have Shay. He called police on me. The police put intervention order on him for assaulting me. (Refer to Order). 2006 - Kevin was in a relationship with Carol Walsh at this time 2006 - Kevin decided to put intervention order on me claiming I was verbally abusing him and his family. We went to court in Ringwood and the judge put an intervention order on both of us with no admittance to doing anything wrong.(Refer to Order) 2007 - Carol Davies/ Walsh is Kevins current partner who was pregnant with her first child to Kevin (Rhiannon) 2007 15th January - Shay began childcare with Margaret Schembri while I was working afternoon shift at Dollar Sweets full time. Things were working out really well, and both Shay and I were very happy. Page 2

2007 18th March after weekend access with her father, Shay wouldnt talk, but clung onto me and cried. She was very withdrawn. She couldnt express herself. I gave her a bath, and she just wouldnt leave me. She wouldnt sleep in her own bed, so I let her sleep in mine. That night she wet the bed, something that she has never done before, as she had been toilet trained since she was two. 2007 21st March - Shay disclosed to Margaret that on the weekend when she was with her father he bit her on her tooter, referring to her private parts. 2007 22nd March - Margaret dropped Shay off to me and told me what Shay had told her. I was devastated and made an appointment with my doctor, Dr Barbra Inness to see Shay straight away. *INSERT REPORT, BARABRA AND DHS* Have a letter from Barbara and what she saw. Barbara then reported it to DHS, who advised me to keep Shay under our protective care while investigation was taking place. 2007 23rd March - Shay and I attended an appointment at SECASA at Monash Medical centre where Shay was examined. Shay was very withdrawn. 2007 26th March - Shay and I attended Dandenong Police Station SOCCA Team where they tried to interview Shay. Shay was too scared to talk about what happened. It was then referred to Ringwood Police to investigate the matter. 2007 3rd April Shay attended her first session with Linda Hansen, the Windermere Child & Family Services Inc Psychologist. I was also present. *INCLUDE REPORT* 2007 3rd April - Margaret made a statement at Dandenong Police Station *SOCCATEAM* in regards to what Shay had disclosed to her. ********************************************************** 2007 4th May My solicitor Sandy Keyser of Duffy and Simon rang DHS and the protective worker said I was acting protectively and no intervention was required. ????? 2007 25th May - Geraldine Noyahr a carer of Shays did an affidavit for the court.

2007 30th May - A letter come from Detective Whitelaw saying that the police had concluded their investigation and were no longer preceding but would keep on record if anything came up in the future. 2007 31st May - The Family court proceedings were adjourned by Justice Brown for a Magellan Hearing on the 8th June 2007 31st May - Teresa Veli did an affidavit 2007 5th June - Family court hearing ???? 2007 8th June - Mark Finn was appointed as Shays Lawyer 2007 14th August Kevin had an appointment with Dr Kennedy for a psychosexual assessment. 2007 16th August Went to city to Alan D Jager to get a Psychiatric Legal report done which was court ordered. 2007 26th September Attended an appointment with Neville Evans Family court report writer in City. Did Shay attend ??? 2007 12th October Kevin began supervision access with Shay on Fridays for four hours at Knoxfield play zone. Maria Angelico was the professional supervisor. 2007 15th November - Received a closing report after 6 psychology sessions with Dr Martin Baker *REFER TO REPORT* 2007 20th November Attended another appointment with Neville Evans Family court report writer in city. Did Shay attend ?? 2007 1st December 9.30am Ken Keerman, a Justice of Peace , who was a coworker at Dollar Sweets took me for a ride in his aerochute where it ended up in him having a crash landing. That left me with severe injuries with my lumbar 1- 4 chipped. I was taken to hospital by ambulance. I spent a week at the Alfred hospital then went to Caulfield for rehabilitation. Shay was with mum and nana for the first week and Margaret brought them to hospital everyday to see me. When they left, Shay went home with Margaret until I went home. Margaret would bring Shay everyday to be with me and she would also help me out. I couldn't move much for a while as I was in a lot of pain. It was a very painful and stressful time for me. Page 4

2008 29th January Margaret took Shay to Narre Warren Police station for access with Kevin but he did not turn up. Margaret got a signed statutory Declaration from the police on duty. 2008 6th February Margaret did an affidavit With Sandra at Duffy and Simon Pakenham *2008 Easter time I had to send Shay away for 6 weeks to friends house for a Counselling Program at SECASSA in Dandenong. I made the heartbreaking decision for Shay to attend the six-week course, hoping that the truth will be revealed. I wasnt allowed to have any contact with Shay at all. Shay had her 5th birthday while she was away and she wasnt allowed to know. That was instructed by DHS. When Shay came back I only had her for a week. While Shay was away, I had the assessment with Dr Entwistle. The report wasnt at all promising or in my favour. I was so traumatised and devastated. ***April 2008 ??? I had an assessment with Timothy Entwistle. This was ordered by the court. *****Shay came back from 6 weeks away

COURT DATE I took Shay to a friends place to be looked after while went to city for court. It was a long day. We waited around got asked lots of questions about where Shay was so that DHS would go and take (abduct) Shay away. She clung on to Claire and cried. She didnt want to go with DHS. Shay was terrified and didnt know what was happening. On the other hand we did not know what was happening and did not know that Shay was being taken away by DHS. Went to DHS in Dandenong after court *** Is this date right ???2008 May Shay was removed from my care while I was in the city at a court hearing. This was purely based on Dr Entwistles report. I had been the Primary carer for Shay from 2003 till 2008. I did not know what was happening. 2008 5th June Went to an appointment with Dr Fred Stamp*** 2008 29th July IAO variation to increase access time with Shay****** Page 5

COURT 5th November - 12th November - Trial in the Childrens Court Melbourne Kevins mother was stated full time carer of Shay, with Kevin to visit under her supervision??? It was a very grievous day for me. ******5th November Mr Ken Kermeen a Justice of Peace, whom I knew from working at Dollar Sweets stood up in court and testified by lying about me due to suing him for injuries he caused to me in the aerochute accident. He kept my personal files of my previous history and wouldnt return them. 25th December 2008 I spent Christmas Day with Shay at Margarets house as I was still suffering from the injuries I sustained in the accident. I couldn't sit in the car for too long to go and visit my family in Ararat and Casterton. ******2008 I attended Shay's Christmas performance at her kinder in Kilsyth with Joy McKerihan. Shay was the little donkey and become embarrassed. She wanted to come to me and half way throughout the performance she ran to me and sat on my knee. She clung on to me and just cried. She didnt want to let go of me. When it all finished she cried even more and wanted to go home with me. It was very heartbreaking for Joy and I to see her so disturbed and it upset me greatly. Joy wrote a letter which is attached herewith. LETTER 2009 30th January there was an attempt by Mr Fraser and his lawyers to stop me attending Shays first day of school. I only got to walk Shay to her class room. Rob Ward was with me due to requiring supervision. This broke my heart that I couldnt be more involved on this very special day. 2009 10th March A Best Interest Plan Case Meeting at DHS in Dandenong. With who ****** REPORT 2009 12th March Letter from Dr Martin Baker clinical Psychologist I was attending. 2009 17th April My sister rang Shay at Kilsyth and was abused by Carol as she was only allowed to speak to Shay when with me. This has made it difficult for my familys communication with Shay. Page 6

2009 5th August did a MMPI -2 assessment with Martin Baker Clinical Psychologist in Pakenham 2009 14th August I successfully completed a Positive Lifestyle Program with the Salvation army which is court recognised. I have certificate. 2009 30th August Had a meeting at DHS in BoxHill not Ringwood with DHS worker Daniella Wilson 2010 11th January I went for an appointment with Family relationships centre in Berwick. Kevin also was asked to attend but he failed to do so. He didnt return the ladys phone calls or letters sent to him. She gave me a certificate of attendance. 2010 DATE ?? Family court in city 2010 10th March A best interest Plan Case Meeting for Shay was held at DHS in Dandenong with Telafulu the DHS worker for Shay. 2010 6th May I attended a second appointment with Dr Entwistle. In his report dated 31st May 2010, page 5 he was of the opinion that some consideration should be given for unsupervised contact with Shay. That never occurred. 2010 May Wasnt that in March??? At the Family Court in the city the Registrar made an interim order till the next Court hearing that Margaret Schembri was not to have any contact with Shay and should the father not come up with evidence against her then the order was no longer valid. 2010 25th July On access I took Shay to Ballarat to see my dad in hospital. Shay was delighted to see dad and talked to him the whole time we were there. 2010 22nd October I rang Shay at Kevins house but Shay wasnt home. He refused Shay to call me back. This happens on numerous occasions and Shay and I are always on speaker so Kevin can listen to our conversations. I have been constantly having trouble with the calls and he has been told he will be recorded. Since this has been put in place, it has not been as intense. 2011 1st February had another Psychiatric assessment with Dr Timothy Entwisle.ELOBORATE Page 7

2011 16th April I had party at home for Shay for her 8th birthday. My dad and his partner together with my nieces and nephew were over to celebrate with Shay. There were also some of her friends and some who are with us at access. We all had a great time. 2011 May Mum and Nana came down and spent Mothers Day weekend with Shay and I. Shay was pleased to learn that she was 4th generation. 2011 21st May Shay and I went to Ararat with Judy, Tynah and Alyssa for my dads partners 60th birthday. Shay saw all her cousins and met lots of other relatives. She was very happy as always.
