MEDICAL AND DENTAL INSTRUCTIONS (present this form to physician, dentist or hospital as authority for requesting treatment) GENERAL 1. If you require Emergency medical/dental care while on leave report to the nearest Canadian Forces (CF) medical/dental facility if possible, otherwise report to a unit in order of priority as follows and ensure that notification is provided as soon as possible. a. b. c. d. any unit of the CF; a physician, dentist or hospital of the department of Veteran Affairs (DVA), notify the nearest CF unit. any unit of another Commonwealth Force, notify the nearest CF unit (in Great Britain the Canadian Defence Liaison Staff (CDLS) London, England; any unit of the forces of the United States of America (USA); notify the nearest CF unit (in the USA, the CDLS Washington, D.C.);

Un militaire qui a besoin de soins mdicaux ou dentaires durgence pendant un cong doit se prsenter la clinique mdicle ou dentaire des Forces canadiennes (CF) la plus rapproche, sinon, se prsenter une unit selon lordre de priorit suivant et sassurer que la service intress est averti le plus rapidement possible: a. b. c. d. e. f. g. 2. If you receive medical/dental care on leave you shall report to your unit medical/dental office on return from leave. NOTE TO CIVILIAN PHYSICIANS, DENTISTS OR HOSPITALS 3. Emergency treatment (e.g. short term illness, relief of pain, repair of broken dentures) may be provided. No elective procedures are to be undertaken. If treatment other than emergency is required, the military patient should be referred to the nearest military facility, or DVA. Your cooperation in notification is requested. A civilian dentist must have prior authorization from the CF dental unit or NDHQ/DGDS before commencing comprehensive treatment, including provision or reconstruction of a denture. ACCOUNTS 4. Except in the case of care received in the USA, accounts for emergency care received in Canada and elsewhere shall be submitted in triplicate on a professional statement, with a descriptive case history attached in duplicate to the patients unit. The address of the unit is on the reverse of this leave authorization or it can be provided by the patient. For treatment in the USA, submit accounts to the Canadian Defence Liaison Staff Washington, D.C. 5. In the case of dental treatment, the member alternately may pay the practitioner and obtain reimbursement by submitting, through his unit a form CF52, General Allowance Claim, supported by an itemized receipt. 6. For dental treatment rendered in Canada, units will forward all accounts to the Dental Unit CO in whose region the treatment occurred, for approval.

2. Le militaire qui reoit des soins mdicaux ou dentaires pendant un cong doit se prsenter au service de soins mdicaux ou dentaires de son unit ds son retour. NOTRE LINTENTION DES MDECINS, DENTISTES ET HPITAUX CIVILS 3. Les mdecins, dentistes et hpitaux civils peuvent donner des soins durgence aux militaires, (dans le cas de brves indispositions pour soulager la douleur ou rparer des prothses dentaires), mais ils ne doivent effectuer aucune opration chirugicale non urgente; il doit tre envoy la clinique militaire la plus rapproche ou une installation du ministre des Affaires des anciens combattants. Nous comptons sur votre collaboration pour nous avertir des cas qui se prsentent. Un dentiste civil doit obtenir lautorisation de la clinique dentaire des FC la plus rapproche ou du QGDN/DGSD avant dentreprendre un traitment complet et mme de foumir ou de rparer une prothse dentaire. COMPTES 4. le militaire doit soumettre son unit le compte relatif aux soins durgence reus au Canada et a ltrager, sauf aux tat-Unis, en trois exemplaires sur un document approprie et accompagn dune observation en deux exemplaires. Laddresse de lunit figure au recto de la prsente formule et peut aussi tre fournie par le militaire. Dans le cas des soins dispenss aux tat-Unis, prsenter le compte ltat-major de liason des Forces canadiennes, Washington, D.C. 5. Dans le cas de soins dentaires, le militaire peut payer le dentiste et se faire gnrale de demande dindemnit avec un reu dtaill lappui.
