Diagnostic Coding on Teleplan Claims/Encounters


1. The minimum reporting requirement is one 3 digit ICD9 code. Reporting the diagnostic code to the 4th or 5th digits is recommended where possible as this will lead to a more accurate calculation of the practices funding level (i.e. more accurate ACG assignments for each patient). To assist in recording the diagnostic code, a list of the 100 most frequently used codes is supplied in Table 1. This list can be used as a short list by the software vendor, or as a pick list in the practice. NOTE: This list of common ICD9 codes is not comprehensive practices are encouraged to use any and all appropriate ICD9 codes, not just the 100 most common codes on this list. The Top 100 list in Table 1 is provided for ease of reference only. Tables 2 and 3 provide similar reference lists of ICD9 codes specific to HIV infection (Table 2) and mental health (Table 3). These codes will be especially useful to the PHCO sites that provide primary care to patients with special needs in these areas. 2. Up to three ICD9 codes can now be reported on a patient encounter record. While only the first diagnostic code is required, it is recommended that all diagnoses established during the patient encounter be reported on the encounter record. Since up to three diagnostic codes can be recorded on the patient encounter record, the order in which the codes are recorded is important. The following can be used as a guide to coding diagnoses: The first (or the Main) diagnostic code is the ICD9 code representing the patients condition established after assessment that is clearly more severe and demanding of resources and that is the primary reason for the patients visit to the practice. The second diagnostic code is the ICD9 code representing a patient condition that coexists with the main diagnosis (e.g. chronic underlying condition, external cause of the condition). The third diagnostic code is the code representing another patient condition identified during the visit at the practice.

Examples of Reporting Diagnostic Codes

1. A healthy patient visits the physician for a routine check-up. Additional Diagnostic Code Reporting Guidelines

The International Classification of Diseases Revision Conference, which is an international meeting of experts on disease and related health problem reporting, has provided the following guidelines for recording diagnostic information: 1. The health care practitioner should select the main condition to be recorded, as well as any other conditions, for each episode of health care. 2. Each diagnostic statement should be as informative as possible in order to classify the condition to the most specific ICD category. 3. If no definite diagnosis has been established by the end of an episode of health care, then the information that permits the greatest degree of specificity and knowledge about the condition that necessitated care or investigation should be recorded. 4. Episodes of health care may occur when someone who may not currently be sick requires or receives limited services. The details of the circumstances should be recorded as the main condition (e.g. immunization). 5. Where an episode of health care concerns a number of related conditions, the one that is clearly more severe and demanding of resources should be recorded as the "main" condition. Where no one condition predominates, a term incorporating multiple conditions (e.g. multiple fractures) may be recorded as the "main" condition. 6. When a condition of external causes is recorded, it is important to describe both the nature of the condition and the circumstances that gave rise to it. (e.g. open wound of the chin - patient fell from bicycle and hit chin on the curb). 7. Where an episode of care concerns a residual condition of a disease that is no longer present, the sequela should be fully described and its origin stated with a clear indication that the disease is no longer present. Where an appropriate ICD9 code exists, the ICD9 code should be used, although the MSP Internal codes (in the table below) are still accepted:
