Prenatal Vitamins

Contains high levels of folic acid, iron, and calcium. Folic acid- reduces the risk of having a baby with a serious neural tubal birth defect. Calcium- prevents a new mother from losing her own bone density as the fetus uses the mineral for bone growth. Iron- helps both the mother and baby's blood carry oxygen. Side Effects: nausea

Zidovudine

Indications: used in HIV infected women to prevent the virus from being passed to their babies during pregnancy and delivery. Side effects- pale skin, unusual tiredness or weakness, fever, chills, or sore throat, headache, trouble sleeping, muscle soreness, and nausea. Nursing Implications: monitor viral load and CD4 counts, monitor CBC

Deferoxamine

Indication: used to remove excess iron from the body, antidote. Side Effects: Bluish fingernails, lips, or skin; blurred vision or other problems with vision; convulsions (seizures); difficulty in breathing (wheezing), or fast breathing; fast heartbeat; hearing problems; pain or swelling at place of injection; redness or flushing of skin; skin rash, hives, or itching Nursing Implications: monitor signs of iron toxicity, monitor vital signs, intake and output, serum iron

Reglan

Indication: used in treatment of GERD, anti nausea. Hiccups. Side Effects: Decreased energy; diarrhea; dizziness; drowsiness; headache; nausea; restlessness; tiredness; trouble sleeping, NMS, reglan can be found in breast milk. Nursing Implications: assess for extrapyramidal side effects, monitor for tardative dyskinesia.

Rhogam

Indications: used to protect an Rh-positive fetus from antibodies by its Rh-negative mother. Side Effects:anaphylaxis, systemic reaction, swelling, induration, redness and mild pain at the site of injection. Nursing Implications: RhoGAM should be administered within 72 hours of delivery or exposure to Rh-positive red blood cells.

Aquamephyton (Phytonadione)

a vitamin, which is a clear, yellow to amber, viscous, odorless or nearly odorless liquid. Indications:coagulation disorders; prophylaxis and therapy of hemorrhagic disease of the newborn Side effects: anaphylaxis, Pain, swelling, and tenderness, "flushing sensations" and "peculiar" sensations of taste, dizziness, rapid and weak pulse, profuse sweating, brief hypotension, dyspnea, and cyanosis. Nursing Implications: Inject slowly, no more than 1mg/min, Benzyl alcohol as a preservative has been associated with toxicity in newborns. Newborn dosage: 0.5 mg - 1 mg SC or IM

Triple Dye

Umbilical area antiseptic. Indications: reducing Staph colonization in newborns, umbilical cord treatment. Side Effects: can cause skin necrosis if it is inadvertently applied to the skin surrounding the umbilical stump; with extended use: nausea, vomiting, diarrhea, and mucosal ulceration Nursing Implications: When applying to cord stump, avoid inadvertant application onto abdominal wall.

Hepatitis B

Indications: vaccine, not contraindicated in pregnancy. Used to prevent transmission of HBV to the infant. Nursing Implications: teach to avoid alcohol, avoid hepatotoxic drugs (ieAcetaminophen)

Ferrous Sulfate (FeSO4)

Indications: treat or prevent iron-deficiency anemia, may reverse gastric, esophageal and other tissue changes caused by lack of iron. Side effects: nausea, heartburn, anorexia, constipation, diarrhea, black stools Nursing Implications: monitor Hgb, continue iron therapy 2-3mos, monitor BMs, monitor sleep patterns dose: max is 15mg/day

Yutopar (Ritodrine)

Indications: used to stop premature labor Side Effects: chest pain, tightness, shortness of breath, dizziness, drowsiness, dry mouth, tachycardia, nausea Nursing Implications: ECG before therapy, validate the woman is in PTL and greater than 20wks and less than 35 weeks gestation, have Inderal available to reverse adverse reactions r/t cardiovascular function.

Terbutaline

Indications: relax smooth muscles and inhibit uterine activity. Stops preterm labor. Side Effects: SOB, coughing, tachycardia, palpitations, chest pain, hypotension, tremors , fluid retention, headache, hyperinsuliemia, hyperglycemia, N/V, fever. Fetal: tachycardia, hyperinsulinemia, hyperglycemia Neonatal: hypoglycemia, hypocalcemia, hypotension, ileus. Nursing implications: ECG before therapy, validate woman is in PTL and over 20wks and less than 35 weeks of gestation. Obtain baseline of fetus and mother. Have Inderal available to reverse adverse reactions.

Nubain (opioid agonist-antagonist)

Indications: moderate to severe labor pain and postoperative pain after cesarean birth. Side effects: confusion, sedation, drowsiness, headache, dizziness, nervousness, sweating, resp depression, N/V, bradycardia or tachycardia. Nursing Implications: may percipitate withdrawal symptoms, assess for respiratory depression.

Stadol (opioid agonist-antagonist)

Indications: moderate to severe labor pain and postoperative pain after cesarean birth. Side effects: confusion, sedation, drowsiness, headache, dizziness, nervousness, sweating, resp depression, N/V, bradycardia or tachycardia. Nursing Implications: may percipitate withdrawal symptoms, assess for respiratory depression.

Demerol (opiod agonist)

Indications: moderate to severe labor pain and postoperative pain after cesarean birth. Side Effects: N/V, sedation, confusion ,drowsiness, tachycardia or bradycardia, hypotension, dry mouth, urinary retention, resp depression, decreased FHR Nursing Implications: assess pain, V.S., FHR, respiratory status, Naloxone is antidote.

Macrobid (nitrofurantoin)

Indications: UTIs Side Effects: dizziness, drowsiness, headache, chest pain, anorexia, N/V, diarrhea, hypersensitivity reactions. Nursing Implications: Assess for s/s of UTI, obtain specimen, monitor I&Os, monitor CBC
