Bernadette Manasan Pollution Control Officer Proposal for the Conduct of Source Emissions Testing

Dear Engr. Manasan, The GEOSPHERE Technologies, Inc. is pleased to submit this proposal for the conduct of source emissions testing for Anson Building located at 23 ADB Ave. Ortigas Center, Pasig City. Source testing will be conducted to monitor the levels of NOx and CO emissions from two (2) generator sets in three (3) runs per parameter at three (3) trials per run for NOx. Collection of samples will be done using grab sampling employing Method 7 for NOx and Method 10 for CO. Collected samples will be analyzed at a DENR-recognized laboratory using standard methods. Monitoring report will be submitted to Anson Building in two (2) weeks after sampling works and results of measurement will be compared to DENR Standards. For and in consideration of the above-mentioned scope of work, the total compensation amounts to Thirty Seven Thousand (PhP 37,000.00) Pesos Only excluding VAT. The terms of payment will be as follows: 50% - Upon issuance of Notice to Proceed or P.O. 50% - Upon submission of report to Anson Building Other terms and conditions for the implementation of the work shall include: 1. Anson Building shall: Issue a Notice to Proceed or Purchase Order (P.O.) at least two (2) weeks before the date of actual sampling Notify GEOSPHERE of the approved P.O. in advance for the preparation of the required source testing plan to be submitted to EMB-NCR;

 Provide sampling ports/ port holes (3 diameter) for each stack and safe access to the platform for the Sampling Team. For safety, the integrity of the ladder and platform shall be verified/inspected and confirmed by the Safety Officer of Anson Building prior to its use; Provide 220V power supply for the sampling equipment, lighting at the workplace and water supply for cleaning of equipment; Designate a Sampling Coordinator/Safety Personnel to facilitate the issuance of relevant permits and gate passes prior to and/or after sampling works; Issue a certificate of completion to GEOSPHERE after sampling. 2. The GEOSPHERE shall: Prepare and submit the required source testing plan to EMB NCR. Observe the companys safety rules and regulations to prevent any accidents or untoward incidents that may cause harm to both parties; Observe and maintain good housekeeping at all times while inside the compound of Anson Building. Ensure integrity of samples during collection and transport to DENR recognized laboratory; Treat the report of analysis and sampling report as confidential and deliver to Anson Building in two (2) weeks after sampling works.
