Registration Information
Only one form per registrant; duplicate as necessary. Mastercard, Visa, and checks made payable to the Kansas Dental Association are accepted for payment. An entire office can send their forms together and make one payment. A $40 late fee will be charged for EACH registrant after May 28, 2012 EXCEPT for dental spouses/guests and dental students. A $50 processing fee will be charged for each on-site registration EXCEPT for dental spouses/guests and dental students.

Hotel Reservations
Room reservations for the meeting will be made by individual attendees directly with the Hotel at Oldtown reservations at (877) 265-3869. All reservations must be guaranteed with a major credit card. Reservations must be received on or before 5:00 PM, Tuesday, May 15, 2012. After this time, reservations will be based on a space-and-rate-available basis. The KDA does not reserve or put a hold on extra rooms. If any meeting attendee books a room outside the group block through a travel agency or discount site, please let a KDA Staff person know so we can have it credited towards the KDAs block.

Presidents Dinner Party

The KDA Presidents Cocktail Reception and Dinner Party on Friday, June 8 is open to all meeting attendees and you do not have to be registered for the meeting to attend. Business attire is preferred.

About the Speakers

Laney Kay, JD of Entertaining Training, LLC. is a nationally known speaker and writer on technical and regulatory topics and womens issues. Her expertise is in taking very complex, and/or incredibly boring, topics and making them both fun and informative. She is a featured consultant with the ADAs CELL Seminar Series and has taught courses at multiple Hinman and ADA meetings, the TBSE, the Yankee Dental Congress, the Chicago Midwinter Meeting, the Pacific NW Dental Conference, the Texas Meeting, the SW Dental Conference, plus other national, state and district meetings, study clubs and in individual offices all over the country. Dr. Kaneta R. Lott has helped to shape the smiles of hundreds of children since 1984. She earned her Doctor of Dental Surgery from Emory University and completed her residency at Emory University and Scottish Rite Hospitals. Her practice is in metropolitan Atlanta. A renowned national speaker and motivator, Dr. Lott lectures nationally on Pediatric Dentistry and Dental Traumatology. She is a consultant to the Georgia Board of Dentistry and the American Dental Association and has served as an Assistant Clinical Professor in the department of Pediatric Dentistry at The Medical College of Georgia. Dr. Lott is also a fellow with the American College of Dentists and The International College of Dentists. She is a member of numerous local, state, and national professional and civic organizations.

Robert Marbach, BA, CDT is a Past Board of Directors at The Pankey Institute. He is Founder of Authentic Dental Lab in San Antonio, TX. Mr. Marbach is a Board Certified Dental Technician currently working on his Masters in dental technology and has over 25 years of dental experience. In 1999 Bob received his Technical Certificate of Completion from the Pankey Institute. Mr. Marbach is a past member of the Advisory Board for The Canadian Journal of Dental Technology. He is also a member of the prestigious TEREC Group, a technical research consortium. Bob has lectured internationally for many years and his lecture has received approval from the Academy of General Dentistry. He has also received his Fellowship in the Academy of Dental Facial Esthetics and received the National Association of Dental Laboratories Inventors Award for developing and patenting the Shade Wand, a shade-taking light used by dentists worldwide. And, in 2011 he was inducted into the American Society For Advanced Dental Esthetics. Corky Willhite, DDS has achieved Fellowship by the Academy of General Dentistry and the American College of Dentists. He is one of 50 Fellows of over 6,000 American Academy of Cosmetic Dentistry members worldwide and has served on their Board of Governors (currently American Board of Cosmetic Dentistry) and spent years as an Examiner for Accreditation. He is on the faculty of the Center for Esthetic Excellence in Chicago and post-graduate esthetic programs at Eastman Dental Center and the University of Minnesota. His private practice in suburban New Orleans, The Smile Design Center, is limited to Cosmetic Dentistry.

One child per classroom may experience a traumatic dental injury. The primary care dentist needs to act quickly and skillfully. Triage treatment determines the prognosis. The program is designed to improve the participants ability to treat a traumatic dental injury and is designed for all attendees. Each participant will receive: Pre-packaged supply list Listing of the necessary radiographs Full understanding of the healing process Step-by step evaluation approach Logical evaluation form that reviews: Extra and intra-oral soft tissues, Facial and dental bones, Periodontia and tooth structures Listing of the necessary clinical tests Specialist referral list Timetable for follow-up treatment

This course will explore the additional skills needed to provide dental care to the pediatric patient. It will also improve the confidence of the staff and improve their rapport with the parent. Celebrate 140 Years of Kansas Dental Association

Please join us for an evening to celebrate the Kansas Dental Association and the 140 Years, 140 Stories project. Over the past year, the KDA has traveled the near and far corners of the state, photographing member dentists. Weve focused on unique interests, hobbies, and distinct locations in which they practice. These photographs will be on display at the KDA Presidents Cocktail Reception and Dinner, when we will unveil our photos to the KDA Membership. This compilation of photos will be arranged into a coffee table book later this year to be made available to KDA members. Each dentists photograph in the book will be in black and white and each are holding a number from 1 to 140 which corresponds to their page location in the book. These photographs and stories are an incredible array of passions and histories unique to each dentist, while demonstrating the common energy and passion of our great state. Please join us to celebrate the book during the Kansas Dental Associations Presidents Cocktail Reception and Dinner.

Join your colleagues for an evening of friendship and commemoration and raise your glass to a successful year, as Dr. Hal Hales leadership term comes to an end. Two of your colleagues will also be honored for their contributions to organized dentistry. Dr. Brett Roufs will be presented with the Dentist of the Year Award and Dr. Mark Herzog with the Dr. Harry M. Klenda Award for Outstanding Council/Committee Service Award. Plan to be a part of this distinguished celebration. Business attire is recommended for the evening and guests do not have to be registered for the meeting to attend.
