ALZHEIMERS ASSOCIATION NORTHERN CALIFORNIA & NORTHERN NEVADA POSITION DESCRIPTION

POSITION TITLE: Family Care Specialist (Brain Health Center) DEPARTMENT: Program REPORTS TO: Chief Program Officer (with dotted line reporting to Dr. Catherine Madison at CPMC Brain Health Center) Hours : Exempt, Full-Time, 40 Hours per week Location: Primary location - CPMC Brain Health Center, San Francisco, CA POSITION SUMMARY Under the direction of the Alzheimers Association Chief Program Officer, the Family Care Specialist is responsible for providing direct assistance and professional intervention to Alzheimers families and to foster cooperative relationships with the infrastructures of local community health and social service professionals. This full time exempt position will be housed at CPMCs Brain Health Center in San Francisco and will be responsible for meeting the goals and objectives outlined in that collaborative project. ESSENTIAL JOB FUNCTIONS

Patient & Family Services Provide consultation to individuals and families (needs assessment, care planning, resource referral, advocacy as needed, follow-up and documentation), in-office, by telephone and, if needed by home visitation
Coordinate with interdisciplinary team at the BHC to link families to community resources such as daycare, homecare, elder law attorneys and more.

Support Group Programs Coordinate and facilitate Early Stage Support Group and caregiver groups at Brain Health Center Community Education & Outreach Provide three memory caf workshops for recently diagnosed individuals and their caregivers to learn more about brain health and memory tips.

Provide ongoing monthly workshops on topics such as Maximizing Your Memory, Healthy Body, Healthy Brain, The Latest in Alzheimers Science and Care and Know the Ten Signs of Alzheimers Provide a minimum of twice yearly series of the Savvy Caregiver Classes. Provide quarterly Caregiver University classes on topics such as Compassionate Communication and Dealing with Difficult Behaviors, Moving a Loved One with Memory Loss and other topics as requested by caregivers using the center support groups and workshops. Other Responsibilities Gather necessary client data for funders Other duties as assigned by Chief Program Officer in collaboration with Brain Health Center leadership Actively take part in Walk to End Alzheimers and advocacy related events in the local community KNOWLEDGE, SKILL REQUIREMENTS and EXPERIENCE Masters in Social Work, Family Counseling, Gerontology or related health care field LCSW, LMFT or Ph.D. Licensure Preferred 2 years experience in geriatric and/or Alzheimers care Record of speaking and writing on dementia and caregiver issues Care management and case documentation skills; ability to monitor and evaluate care plans, and refer to resources, ability to respond to emotional concerns of family members Dynamic public speaking ability Computer: word processing and database management Excellent verbal and written communication skills Experience working with diverse communities Bi-lingual skills desired, especially Spanish or Chinese. PERSONAL CHARACTERISTICS Self-starter, independent worker and willing to take initiative Willingness to work some evenings and weekends Willingness to travel in service area Desire to help families cope with Alzheimers disease and related dementias.
