RECEPTORES SENSORIALES EN MSCULOS, ARTICULACIONES Y RGANOS VISCERALES. La propiorecepcin es la informacin de la posicin del cuerpo y el movimiento en el espacio, generalmente no alcanza niveles corticales y no contribuye a la percepcin general, sin embargo, la informacin propioceptiva es fundamental en la coordinacin voluntaria y el movimiento reflejo del msculo esqueltico. Existen dos tipos de receptores en la propiocepcin: A) Husos Musculares, monitorea el grado de estiramiento del msculo inervado. B) rgano Tendinoso de Golgi: Monitorea la tensin del msculo. Tambin existen mecanoreceptores en el conectivo de las articulaciones que hacen propiocepcin, muchos son de adaptacin rpida (movimientos de extremidades), lento adaptacin (posicin del cuerpo). Sensaciones del msculo liso y por rganos internos son mucho menos discretas que las de estructuras somticas, como la piel y el msculo esqueltico. Tambin contribuye a los estmulos de hambre y sed, y al bienestar y el malestar. La propiocepcin hace sinpsis con las ramas somatosensoriales generales, entonces se pueden tener malestares propioceptivos reflejados en la piel.

PROYECCIONES CENTRALES GENERALES DEL SISTEMA SOMATOSENSORIAL


PROPIEDADES GENERALES DE LAS RUTAS DE TRANSMISIN CENTRAL Los receptores convierten la informacin de estmulos qumicos, trmicos y mecnicos (seales bioelctricas) en potenciales de accin y de generador. Los potenciales de generador son respuestas localizadas a un estimulo sensorial que varan en intensidad basado en la intensidad del estmulo (los potenciales de generacin de suficiente tamao, generan un potencial de accin). Los potenciales de accin no son respuestas localizadas, en cambio ellos se propagan por las fibras de proyeccin central de la neurona. En un potencial de accin la modalidad es la respuesta preferencial de una neurona sensorial a un estmulo dado. La ubicacin es delimitada por el campo receptivo de la neurona activada, y la intensidad es la frecuencia de potenciales de accin. Las neuronas que se adaptan rpido, slo transmiten informacin de un cambio de estmulo (novedad), y se activan cuando el estmulo comienza o termina. Los cuerpos celulares de las neuronas somtico-sensoriales estn ubicadas en el ganglio de la raz dorsal, y son denominadas Aferentes Sensoriales de Primer Orden y constituyen la ruta sensorial primaria. Las fibras centrales de esta ruta proyectan con pequeas races dorsales a los blancos sinpticos Neuronas Sensoriales de Segundo Orden, ubicadas en areas designadas de la mdula espinal y el tronco enceflico. Los cuerpos celulares de todos los tipos de los nervios somtico-sensoriales estn agrupados juntos en el ganglio de la raz dorsal, sin embargo, sus proyeccines centrales mantienen distintas posiciones separadas en la espina dorsal. Existen dos rutas sensoriales principales: una que transmiten informacin sobre el tacto y la propiocepcin y la otra transmite informacin sobre el dolor y la temperatura. SPECIALIZED FEATURES OF ROD AND CONE PHOTORECEPTORS


The two types of retinal photoreceptor cells, rods and cones, are not evenly distributed across the retina (Fig. 3). Cones are concentrated in the foveal region, whereas rods are absent. Several of the cellular characteristics of cones make them particularly suited for high-acuity vision. Because of the tightly packed arrangement of foveal cones, the foveal region is capable of high-resolution (finegrained) responses. In addition, the outer segment, which contains most of the visual pigment of the cone, is relatively short and is most sensitive to direct axial rays of light. The photochemical cascade in cones is very fast, allowing the foveal region to have a very rapid response time. Cones are adapted for color vision. Three types of cones have been described: one containing visual pigment that is most sensitive to blue light, one that is sensitive to green light, and one that is most sensitive to long

REFLEJO DE ACOMODACIN (REFLEJO DE CERCANA) El reflejo se inicia cuando la atencin visual es dirigida hacia un objeto cercano. La imagen inicial del objeto cercano (desenfocada) es enviada a la corteza occipital por la va visual primaria. Esa informacin es enviada a la regiones superiores del tronco enceflico y al nervio craneal III. La estimulacin del ncleo de Edinger-Westhpal produce contraccin del msculo ciliar, liberando la tensin del cristalino, dndole una forma ms cilndrica (lo que produce ms enfoque del objeto), la estimulacin del nucleo de E-W tambin produce la contraccin del msculo contrictor de la pupila y disminuye el tamao de la pupila. La activacin del nucleo del Nervio III produce que los msculos recto medial se contraigan para converger la imagen y enfocarla a la regin foveal. REFLEJO PUPILAR POR LUZ Es un reflejo que tiene una rama aferente (del nervio craneal 2) y una eferente (del nervio craneal 3). Lo que incluye las siguientes estructuras (contraccin): 1) Clulas ganglionares de la retina: las cuales proyectan bilateralmente al ncleo pretectal. 2) El ncleo pretectal del mescencfalo: el cual proyecta (a travs de la comisura posterior) fibras cruzadas y directas al ncleo accesorio oculomotor (Edinger-Westphal) 3) El ncleo de E-W del nervio craneal III: donde se inician las fibras preganglionaricas parasimpticas, las cuales salen del cerebromedio con el nervio craneal III y hacen sinpsis con las fibras postganglionares parasimpticas del ganglio ciliar. 4) El ganglio ciliar: donde se inician las fibras postganglionares parasimpticas, las cuales inervan el msculo esfnter de la pupila. - Ruta del reflejo de dilatacin pupilar: Es mediada por la divisin simptica del sistema nervioso autnomo. La interrupcin de esta ruta en cualquier nivel causa el Sndrome ipsilateral de Horner. Se incluyen las siguientes estructuras: A) el hipotlamo. Neuronas hipotalmicas del nucleo paraventricular proyectan directamente al centro cilioespinal (T1-T2) de la columna celular intermediolateral de la mdula espinal B) El centro cilioespinal de budge (T1-T2) proyecta fibras simpticas preganglionarias a travs del tronco simptico hacia al ganglio superior cervical. C) El ganglio superior cervical proyecta fibras postganglionarias simpticas a travs de el plexo perivascular del sistema carotdeo del msculo dilatador de la pupila. Las fibras postganglionares simpticas pasan a travs de la cavidad timpica y el seno cavernoso y entran en la rbita a travs de la fisura orbital superior. CENTROS CORTICALES Y SUBCORTICALES DE LA MOVILIDAD OCULAR A) El campo frontal del ojo est ubicado en la parte posterior del giro medial frontal (rea de Brodmann 8) y regula los movimientos voluntarios del ojo. 1) Estimulacin (lesin irritante etc.) causa desviacin contralateral de los ojos (lejos de la lesin) 2) Destruccin causa desviacin conjugada de los ojos (hacia la lesin) B) Campos occipitales del ojo: estn ubicadas en las reas de brodmann 18 y 19 del lbulo occipital. Estos campos son centros corticales de movimientos involuntarios de seguimiento y bsqueda. La estimulacin causa desviacin contralateral conjugada de los ojos.

C) Los centros subcorticales de la mirada conjugada estn ubicadas en el ncleo abducens del Puente troncoenceflico. 1) Recibe inputs del campo frontal contralateral del ojo y se proyecta hacia el msculo recto lateral (ipsilateral) a travs del MLF (fascculo medial longitudinal) al complejo oculomotor del subncleo del recto medial contralateral. D) Los centros subcorticales de la visin conjugada vertical, estn ubicados en el cerebro medio al nivel de la comisura posterior. Es llamado el ncleo rostral intersicial del MLF y est asociado con el sndrome de Parinaud. CORRELACIN CLNICA A) SNDROME DE MLF, o oftalmoplegia internuclear. Es un dao (desmelinizacin) del MLF entre el nucleo abducens y el oculomotor. Causa parlisis del recto medial lo que afecta en la vista conjugada lateral y en la nistagmia horizontal monocular en el ojo afectado (convergencia es normal) este sndrome es comunmente visto en la Esclerosis Mltiple. B) SNDROME DE 1 Y MEDIO. Consite en lesiones bilaterales del MLF y una lesin unilateral del ncleo abducens. En la visin conjugada lateral, el nico msculo que funciona es el recto lateral intacto. C) PUPILA DE ARGYLL ROBERTSON, o disociacin pupilar con luz cercana. Es la falta de una reaccin de miosis a la luz, lo que produce directa y consensualmente la reaccin de miosis a un estmulo cercano (acomodacin-convergencia). Ocurre en la sfilis y diabetes. D) SNDROME DE HORNER. Es causado por la transecin del la ruta oculosimptica en cualquier nivel. Este sndrome consiste en miosis, ptosis y aparente enoftalmia y hemianhidrosis. EL HUSO MUSCULAR Las fuerzas motoras de la nuerona motora inferior provienen de 3 grandes fuentes: A) rutas sensoriales de la mdula espinal y el tronco enceflico que desencadenan aciones reflejas. B) Interneuronas de la mdula espinal que interconectan bancos sinergistas y antagonistas C) Neuronas motoras superiores, en la corteza motora y otras reas motoras del encfalo que proveen comandos complejos. Una de las contribuciones sensoriales ms importantes de la neurona motora inferior est derivada de un rgano terminal ubicado dentro del msculo. Existen dos tipos de terminaciones sensoriales en el tejido muscular: los husos musculares y el rgano tendinoso de golgi. El huso muscular consiste en un grupo de fibras musculaes de 4 a 10 mm encapsulados en una bolsa fusiforme de TC. A diferencia de la transmisin rpida, punto-a-punto de informacin del sistema nervioso somtico, existen otros tipos de circuitos neuronales presentes el sistema nervioso, que operan de manera mucho ms lenta, y tienen un objetivo-blanco mucho ms amplio y menos definido. Estos sistemas estn agrupados como Sistemas Moduladores Difusos. Uno de los mejores ejemplos es el SNA. La naturaleza difusa y moduladora del SNA se ejemplifica en la porcin parasimptica del Vago (X), el cual se origina de un pequeo grupo de neuronas en los ncleos vagales motores del mescencfalo, estas se proyectan en forma de fibras preganglionares que hacen sinpasis con un gran nmero de ganglios parasmpaticos que controlan msculos del esfago, corazn, cavidad peritoneal e intestino. Comparado con el sistema somtico motor y sensorial, el parasimptico vagal evoca respuestas lentas, mantenidas, y controladas con la cantidad --grande de rganos y tejidos que innerva. EMOCIONES RELACIONADAS A LA FUNCIN DEL SNA Las emociones fuertes, generalmente estn asociadas con respuestas autnomas. Las respuestas parasimpticas coordinadas evocan un estado vegetativo que genera un estado de relajacin general y sensacin placentera, las que generalmente estn relacionadas con los comportamientos al comer (ver u oler una comida). Lo opuesto se obtiene por acciones coordinadas del simptico, en este caso el simptico genera comportamientos de proteccin, preparando al individuo para escapar de una situacin peligrosa o bien, mantenerse en un comportamiento agresivo. En casos de estimulacin simptica sostenida por el tiempo, el individuo presentara sentimientos de miedo, ansiedad y enojo. En los dos casos anteriores, los estmulos del SNA fueron externos (peligro, comida etc), sin embargo existen estmulos internos que desencadenan reflejos autnomos (memorias de alguna situacin placentera o ansiosa desencadenan respuestas del simptico o del parasimptico). La fuerte relacin del SNA con las emociones ha dado pie para dos teoras opuestas que explicaran la experiencia emocional al miedo: 1) La teora de James-Lange, la ms tradicional, explicara que el miedo es experimentado en respuesta a la percepcin del miedo y la activacin de reas corticales apropiadas, lo que originara el desencadenamiento de la actividad simptica. 2) La teora de Cannon-Bard, sugiere que la presentacin de un estmulo que da miedo, se expresara primero que la respuesta simptica, este cambio del tono simpticoparasimpptico sera monitoreado por centros superiores en el SNC, y este cambio del tono sera la experiencia del Miedo, en otras palabras el estado simpattico por si mismo es desencadenado por la emocin.

El circuito de papez representa a un sistema de emociones responsable de unir la experiencia de una emocin con su expresin fisiolgica. El sitio propuesto para la experiencia de una emocin es el Cngulo, la cual recibe informacin sensorial canalizada directamente por el tlamo de reas sensoriales de la corteza cerebral, la salida del cngulo es a travs del hipocampo y se proyecta a travs de una va de fibras denominada formix, al hipotlamo, donde se traduce en la expresin de una emocin. Como el hipotlamo proyecta hacia el nucleo anteorior del tlamo (tracto mamilotalmico), la expresin de una emocin puede aadir un ciclo de feedback positivo a la experiencia emocional (aumentar la emocin). El hipotlamo y su rea asociada, la amgdala (ncleo almendrado, ubicado en el borde anterior del hipocampo con el lbulo temporal), realizan un importante rol en la mediacin del comportamiento agresivo, se demostr en animales, que la ablacin bilateral de la amgdala reduce el miedo y disminuye las formas agresivas, en humanos el dao a la amgdala denota una prdida de la capacidad para detectar el miedo, por el contrario los estmulos elctricos demuestran una exageracin de la ansiedad y el miedo. Estos efectos en la amgdala son mediados a travs de conexiones con el hipotlamo, donde se generan dos tipos distinos de comportamientos agresivos, la agresin afectiva (proteccin familiares cras etc) y la agresin predatoria (al buscar una presa etc.). La amgdala estimula la regin medial del hipotlamo produciendo agresin afectiva e inhibe la del hipotlamo lateral que produce agresin predatoria.

SISTEMAS MODULARES DIFUSOS ASCENDENTES El paquete medial del procensfalo es uno de los tres sistemas moduladores difusos, que ascienden en el tronco enceflico. Contienen fibras que liberan norepinefrina, serotonina y dopamina que estn distribudas ampliamente por los sitios de inervacin del cerebro. La respuesta a la presentacin abrupta de unnuevo estmulo, como un ruido fuerte, supone una reorientacin inmediata de la cabeza y tal vez del cuerpo, hacia el estmulo inesperado y un aumento general de la conciencia y la concentracin.

SUEO
El sueo es la suspencin normal de la consciencia y desde el punto de vista electrofisiolgico por criterios de ondas enceflicas. El sueo en s no supone una disminucin de la actividad cerebral (en el sueo REM el cerebro est igual de activo que en la vigilia). CICLO CIRCADIANO DE SUEO Y VIGILIA El sueo humano se desarrolla con periodicidad circadiana, generalmente relacionadas al reloj biolgico, el cual se entrena--fotoregula, gracias a las detecciones de la disminucin de la luz a medicia que se aproxima la noche. Los receptores que sensan estos cambios de luz se encuentran en la capa nuclear externa de la retina, cuyo pigmento, melanopsina es depolarizado por la luz. Evidentemente la funcin de estos fotorreceptores distintivos es codificar la iluminacin del ambiente y regular el reloj biolgico. Esta regulacin se logra por medio de axones que se encuentran en el tracto retinohipotalmico, que se proyecta hacia el ncleo supraquiasmtico (NSC) del hipotlamo anterior, sitio del control circandiano de las funciones homeostticas. La activacin del NSC, evoca respuestas en las neuronas cuyos axones hacen sinpsis primero en el ncleo paraventricular del hipotlamo y descienden hasta las neurona simpticas preganglionares en la zona intermediolateral en las astas laterales de la mdula espinal torcica. Estas neuronas preganglionares modulan las neuronas de los ganglios cervicales superiores cuyos axones postganglionares se proyectan hacia la glndula pineal, la cual sinstetiza la neurohormona promotora del sueo melatonina a partir del triptofano, la cual es secretada al torrente sanguneo dode modula los circuitos del tronco enceflico que terminan por gobernar el ciclo sueo vigilia. En el NSC tambin gobiernan otras funciones sincronizadas con el ciclo sueo-vigilia como la tempreratura corporal, la secrecin de hormonas (x ej cortisol), la presin arterial y la produccin de orina (en adultos disminuye la prod. De orina x la regulacin circadiana de la ADH). ESTADIOS DEL SUEO El sueo comprende diferentes estadios que se desarrollan en una secuencia caracterstica. 1) Sueo NO-REM (sin movimiento ocular rpido) Sueo estado I: Durante la primera hora despus de acostarse los humanos descienden en los estadios sucesivos del sueo. Estos estn definidos fundamentalmente por cristerios de EEG. Al inicio durante la somnolencia el espectro de frecuencias del EEG se desplaza hacia los valores ms bajos y la amplitud de las ondas corticales aumenta ligeramente. Sueo estado II: Despus del la primera fase, da a lugar al estado II, que se caracteriza por una disminucin mayor de la frecuencia de las ondas del EEG, y un aumento en su amplitud, junto con grupos de puntas intermitentes de alta frecuencia (husos del sueo), los cuales son descargas peridicas de actividad de 10-12 Hz que duran 1-2 segundos y surgen como resultado de las interacciones entre las neuronas talmicas y corticales. Sueo estado III: Representa el sueo moderado a profundo, la cantidad del huso disminuye, mientras que la amplitud de la actividad del EEG aumenta an ms y la frecuencia sigue cayendo.

Sueo estado IV: Es el nivel de sueo ms profundo, tambin conocido como sueo de ondas lentas, la actividad del EEG predominante consiste en fluctuaciones de alta amplitud y frecuencia muy baja (0,5-2 Hz) denominadas ondas delta. Toda la frecuencia desde la somnolencia hasta el sueo IV suelte tardar una hora. 2) Sueo REM (con movimiento ocular rpido), luego de salir del sueo IV se empiezan a invertir los registros del EEG, y se asemejan a los niveles de la vigilia. Despus de unos 10 minutos de sueo REM el encfalo atraviesa de nuevo estadios de sueo NO-REM, en general el sueo de ondas lentas se desarrolla otra vez en la segunda ronda de este ciclo contnuo, pero habitualmente no se produce durante el resto de la noche. En resumen las 8 horas tpicas de sueo que se experimentan en la noche correponden a varios ciclos que alternan entre sueo no-rem y sueo rem, y el encfalo se encuentra muy activo durante gran parte de este periodo. La cantidad de sueo REM disminuye conforme a la edad. CAMBIOS FISIOLGICOS EN LOS ESTADIOS DEL SUEO Los periodos de sueo no rem se caracterixzan por movimientos oculares lentos de rotacin y por disminucin del tono muscular, los movimientos corporales, la frecuencia cardaca, la respiracin, la presin arterial, el ndice metablica y la temperatura. Todos estos parmetros alcanzan sus valores mnimos durante el sueo estadio IV. Por el contrario, los perodos de sueo REM se acompalan por aumento de la presin arterial, la frecuencia cardaco y el metabolismo hasta niveles casi tan altos como los del estado vigil. En el sueo REM ocurre parlisis de muchos msculos grandes, contraccin pupilar, contraccin de msculos de los dedos de las manos y pies y el odo medio, tambin se desarrolla una ereccin peniana espontnea durante el sueo rem. El sueo REM se caracteriza por actividad onrica, y las respuestas motoras son menores (sonambulismo y hablar en sueos se realiza en sueos no-rem). QU ES EL APRENDIZAJE Y LA MEMORIA? RESUMENHabituation, perhaps the simplest -form of nonassoFISIOLOGA JOHNSON PEP 3 - ANBAL ciative learning, refers to a decrement of responsiveness caused by repetition of a stimulus. It is generally distinguished from simple fatigue, as responsiveness can be rapidly restored (dishabituated) by the presentation of a novel stimulus to the animal. The parametric features of habituation have been described by Thompson and Spencer (1966).

Classical conditioning is an example of associative DANIEL PAREDES MERINO - MEDICINA USACH learning in which the presentation of a reinforcing (unconditioned) stimulus is made contingent on that of a preceding (conditioned) stimulus. The change in behavior produced by repeated pairing of the two stimuli can be measured in a number of ways. An example of classical conditioning involves the training procedure originally described by Pavlov (1927) to condition salivation in dogs (Fig. 3). Before training, meat powder (referred to as the unconditioned stimulus, or US) reliably elicited salivation (referred to as the unconditioned response, or UR). The signal for the presentation of the US is called the conditioned stimulus (CS). In the original experiments of Pavlov, the CS was a bell. Traditionally, the CS does not evoke a response similar to the UR and is typically referred to as a neutral stimulus. During training, the US was made contingent on the CS by repeatedly pairing the presentations of CS and US. After training, the response to the CS alone had changed such that the bell elicited salivation (the conditioned response, CR). The persistence of a CR after training is called retention. When the contingency between CS and US was eliminated by repeatedly presenting the bell in the absence of the meat powder (US), the ability of the CS to elicit the CR (salivation) gradually diminished. This process is called extinction.

CONDICIONAMIENTO OPERANTE Correlaciones neuroales del condicionamiento operante han sido encontradas en una neurona involucrada en la generacin de programas de alimentacin motores.
