Proper documentation of the risk analysis is necessary to ensure that all stakeholders understand the scope, method, limitations and conclusions of the risk analysis.

Chapter 15
Documentation is the most important step of the fire risk assessment process because of the following: A record is made of all of the analysis and outcomes of the assessment. All of the stakeholders are informed of the outcomes. Any necessary solutions or comprises are specified. Further changes to the building can be modified by reviewing the original assessment documentation. All of the stakeholders understand the impact of the assessment.

Chapter 15
Fire service / fire prevention officers

This chapter is also the most important for a fire service / fire prevention officer. A fire service / fire prevention officer needs to review all project information to ensure that concerns are addresses. They also need to understand the assessment so any inspections of the building occur in accordance with the assessment. A fire service / fire prevention officer needs to ask for clarification when needed to fully understand the assessment.

Chapter 15
Review

This chapter is important for all of the stakeholders. All of the stakeholders need to review all project information to ensure that concerns are addresses. They also need to understand the assessment so any future work on the building occur in accordance with the assessment. Stakeholders need to ask for clarification when needed to fully understand the assessment.

Chapter 15
The project documentation serves a number of functions, which include the following: Communication of the risk associated with a design or facility to project stakeholders. A basis upon which enforcement officials, peer reviewers, and other stakeholders can review the risk analysis. Evaluation whether changes to a building or facility for which a risk analysis was performed would change the basis for the risk analysis.

Chapter 15
Documentation for a fire risk analysis can include two major elements: The FRA Concept Report - to document the planned approach for the fire risk analysis. Comprehensive project documentation provides complete documentation of the fire risk analysis, which may include part or all of the concept report.

Chapter 15
The purpose of the FRA Concept Report is to facilitate agreement on the approach that is proposed for the risk analysis prior to beginning detailed work. The FRA Concept Report includes information regarding the project scope, risk acceptability threshold, hazards, scenarios considered, and the scenarios that are proposed to be used in the analysis.
9

Chapter 15
The FRA Concept Report presents a roadmap for the analysis, and therefore provides much of the input for the documentation of the fire risk analysis report. This includes updates to the analysis and any results that were generated during the conduct of the risk analysis.

10

Chapter 15
Comprehensive project documentation should be provided on the following: Set of conditions under which the risk assessment is considered to be valid. What types of changes in conditions would require a new risk assessment. Any conditions that are placed on the analysis or assumptions that are made. Any limitations of analysis methods that are used. Whether these limitations of analysis could impact the continued validity of the risk analysis.
