Main Features of the Scheme:


This is a group policy and will commence very soon. The most likely commencement date is 01-12-2011 Members upto 85 years are eligible Join in without undergoing any medical test Sum insured ranges from Rs. 150,000/= upto Rs. 1,000,000 (6 options are available) which covers hospital expenses Additional Rs. 500,000/= cover for 25 critical illnesses for no increase in premium Personal accidents, eye care, child births, deaths etc., also covered for no additional premium Treatment taken in Private and Govt. hospitals as well as Aurvedic hospitals are accepted Spouse and the children also covered but no added premium Members working abroad can insure the spouce and children or parents if they are living in Sri Lanka. Now you also can join this health insurance scheme offered by the NITF You can select one out of 6 options given for the Sum Insured. Age limit upto 85 years This group policy will commence very soon. Therefore submit your proposal form without delay.

What you have to do: in order to join Fill up a proposal form* and hand it over to the IESL finance dept. together with the applicable annual premium as per schedule 2 given in this flyer at least one week before the commencement date. Keep a copy of the document

in case of hospitalization
Just produce the plastic card issued to the insured by the NITF to the hospital reception. Also inform the NITF through the contact no 0718693449 - Mr. Nishantha or 0114873900 upto 5 - NITF general line

at the time of discharge Before two hours of the discharge of the patient, inform the NITF through the contact no as above. (the hospital will do the same and then the NITF undertake to settle your bill subject to the limits as given in schedule 1) If you have exceeded your limits you have to pay only the balance before you discharge when you want to make a claim* Fill up a claim form and deliver/ hand it over to the NITF together with the documents requested within 90 days of the event occurrence. Keep a copy of it

The eligible person includes: 1. The member (of age between 18 years to 85 years) of the IESL who are covered by the valid insurance policy issued by the NITF and the dependents of the member 2. If the IESL member is married, the eligible dependents shall be the spouse of the member (of age between 18 years to 80 years) and the unmarried children up to 21 Years. 3. If the IESL member is unmarried, the eligible dependents shall be the parents of the member up to 70 Years.
