Rental Agreement By placing an order with the House of Glass, you understand and agree to enter into the

following lease terms with the House of Glass: 1. Disclaimer Every effort is made to assure descriptions and prices are correct. The House of Glass reserves the right to correct errors as needed. All prices and availability are subject to change without notice. 2. The Renter MUST deposit a Government issued ID (passport, drivers license, SSS ID or Birth Certificate) in order to avail of our services. Rental period is for the duration of calendar days noted on the invoice. The first day of the rental period begins the day following the first attempt by the parcel carrier to deliver the item. The equipment must be returned to us within the 24hours of the daily rental or within the last day of the weekly rental period. 3. Payment methods: The rent for the Equipment shall be paid in advance in full. We accept Cash, COD, or Bank Deposit. (well be getting a credit card machine soon) We also accept reservation as long as a full payment is made at least 5 days before your desired date of rental. Cancellation of reservations with payment is allowed 2days before the rental date. We shall give you a credit which can be used within a 60day period from date of cancellation. Once the grace period is over the credit may not be used anymore. 4. Shipping, both outgoing and return will be charged P100/way to the customer at the time of the initial transaction and another P100, upon return, granted that the House of Glass is the one who setups the shipping. Shipping is always optional and will only be operating within the boundaries of the National Capital Region, for the meantime. 5. Late returns are charged an additional 1-day rental fee if the equipment is not returned within the 24hr daily rental period or equal to the number of days late. Renter agrees that said fee will be charged when they finally return the item. In the event the rental is not returned 5 days after the due date, the item will be considered stolen and the customers information will be used to conduct a police investigation. If the overdue charges above are denied by the renter, the renter understands the following actions may be taken: a. The Renters information will be turned over to a collections agency, which immediately results in the addition of attorneys and collection fees. These fees cannot be waived and the collection process will continue until these fees are collected, even if the lens is returned. b. Theft charges are filed and a warrant obtained once the account is 30 days old. For equipment of sufficient value our collection agency may employ repossession agencies to collect the original equipment or goods sufficient to repay the value of equipment. 6. Damage or Loss of the equipment during the rental period is entirely the renters responsibility. If the renter has not notified the House of Glass of damage within 3 hours of receiving the equipment, both parties agree the equipment was received in good working order.

a. In the event of loss of the equipment, customer agrees to pay the replacement cost of the equipment to the House of Glass within 3 days or be replaced with the exact same equipment. The customers responsibility ends once they have a confirmation receipt that we have received the package. b. In the event of damage to the equipment, customer agrees to pay actual repair costs at the appropriate factory service center. c. In the event that customer claims the equipment failed rather than was damaged, the report of the factory service center will be considered binding concerning equipment failure or damage. d. Our acceptance of equipment on return is not a waiver by us of any claims for damage to the equipment. e. Equipment will be thoroughly checked to ensure its in top condition for rental. 7. Liability House of Glass does not assume, and the customer indemnifies House of Glass against, any liability or claims resulting from use or malfunction of the equipment. The customer assumes all liability that may arise from use or failure of the equipment. 8. Ownership The equipment remains at all times the sole and exclusive property of House of Lens. The renter has no rights or claims to the equipment. House of Lens does not have or make any claim to images made by the renter while using said equipment. 9. Default If the renter fails to meet any obligation under this agreement, House of Lens has full right to take immediate possession of the equipment; to make a charge to the renter for the replacement value of the equipment; to pursue any legal remedies; and to sue to recover any lost equity or income. In the event of litigation to recover any damages, renter is held responsible for all court costs and legal fees incurred by House of Lens. 10. Severability and Governing Laws This represents the entire agreement between these two parties. If any portion of this agreement is found unenforceable, it will not affect the remainder of the agreement, which shall remain valid and enforceable. The agreement shall be enforced according to the laws of the Republic of the Philippines.

I agree to the terms and conditions laid by House of Glass I agree that my information may/can be turned over to a Collections Agency/Police Authority in the event of my non-return of House of Glass equipment which I rented. I agree to all legal charges that can/may be filed against me by House of Glass and its Owner Mr. Jose Alberto de Guzman
