DISCHARGE PLAN

Discharge planning is the development of an individualized discharge plan for the patient prior to leaving hospital, with the aim of containing costs and improving patient outcomes. Discharge planning should ensure that patients are discharged from hospital at an appropriate time in their care and that, with adequate notice; the provision of other services will be organized.

M Medications The parents of the patient will continue with the prescribed medication with proper dosage, timing and frequency in order to hasten the recovery of the patient. Ampicillin /Sulbactam ( Unasyn) 7.5 mg BID P.O. for 7 days Terbutaline (Bicranyl) 1.5 mg BID P.O for 7 days Prednisolone (Romamed) 1.5 mL BID P.O for 7 days Inform the family members about the prescribed medications and its importance regarding the condition of the patient. Remind and reviewing the family members about the drugs in order to make familiar with the treatment. Advise the family members to avoid using any non-prescription drug unless use is approved by the physician. Encourage the family members to administer medications exactly asprescribed by the physician. Complete the full course of drug therapy especially antibiotics, to prevent drug resistance.

E- Environment Environment should be kept calm and silent so that patient may be able to relax and may have ample time to rest. Emphasize the significant other to avoid patient from crowded and dusty areas. Emphasize the importance of proper ventilation of the room. Encourage safety and security. Emphasize the importance of a safe and clean environment to hasten recovery. T- Treatment Stress to the Significant other of the patient importance of the scheduled follow up check up appointment on _________ to be able to have continuous assessment of condition.

H- Health Teachings . Encourage the mother not to leave her child unattended for safety reasons. Emphasize to the mother the importance of proper hygiene, including hand washing before and after handling the patient to avoid contamination of microorganisms causing further complications. Advice the significant other to keep the patient's back dry at all times. Advise significant other to provide continuous support and care to the patient. Educate the family of the importance of having a regular follow up check up after hospitalization of the patient. O- Observable signs and symptoms If any of the signs and symptoms observed, patient should be able to go immediately to the hospital or nearest health center for further check up. Fever, chest pain, Increased Respirations, Shortness of breath, Difficulty of breathing

D-Diet Encourage the parents to facilitate the patient in increasing the fluid intake to promote hydration. Educate the significant others about foods that are healthy yet appropriate for the diet of the patient. Instruct the significant other to let the patient eat foods rich in Vitamin C to and protein to boost immunity against infection and promote tissue repair. S- Spirituality Stressed to the significant others the importance of taking the medications religiously. Encouraged the family to regularly go to church every Sunday and maintain a good relationship with God through prayers.
