Grading Scale for Clinical Prep

Practical Nurse Program


History and Pathophysiology
o Must define and describe the disease process
o List nursing considerations for a patient with this disease or disorder
o Include patient history
o Must list the reference where information was obtained

Treatments and Therapies


o Must list nursing interventions that are appropriate to this disease process
o List any treatments or therapies this patient has ordered

Nursing Diagnoses and Interventions/ Nursing Care Plan


o List nursing diagnoses appropriate for this patient based on the medical DX with
the 3 part format where applicable (Data collection and Nursing Dx)
o List the overall goal for the nursing diagnosis (Planning)
o List Nursing interventions with scientific rationale that the student will perform
for this patient appropriate to the disease process and nursing Dx.
(Intervention/Implementation)
o List how the student will evaluate the interventions performed (Evaluation)
o List reference where information is obtained

Medications
o List 5 medications that are given to this patient appropriate to the medial DX
(PN 1) OR
o List 5 medications that someone with this disease or disorder would receive
(PN 1)
o Must be able to state common side effects for ALL medication being given
(PN 2,3)
o Must be able to state the 5 Rights of medication administration (PN 2,3)
o Must be able to state and demonstrate proper administration of each medication
being administered (PN 2,3)
o All medication entries must show the following: (PN 1,2,3)
 Trade and generic names
 The dose ordered; look up if this dose is appropriate for the pt.
 Frequency; route
 Classification – functional class and why is the pt. receiving the
medication
 Nursing Implications (ex. Digoxin – get an apical HR first; NTG –
get a BP first and 15 minutes after administration)
Laboratory results and Nursing Notes
o Must list any abnormal test results applicable to this pt.
o If unable to find current labs, describe a lab result which may be abnormal for a
pt. with this disease or disorder. (PN 1)
o Must be able to discuss the reason for abnormal lab results (PN 1,2,3)
o Describe what interventions are appropriate for the lab results (PN 2,3)
o Discuss what the lab test will identify (PN 1,2,3)
o Must include the nursing note for the day and care provided

Clinical Preps are graded weekly. The grading scale is below. Total = 100 points

History 5 points
Pathophysiology 10 points
Treatment 5 points
Laboratory 10 points
Medications 10 points
Nursing note 10 points
Care plan (grid) 50 points
