The following behavioral scale of 5 (excellent) to 1 (poor) will be used to rate your work attitude and behavior in this library work study class. 4-MEETS 2-NEEDS 5 VERY GOOD 3-APPROACHING 1-POOR EXPECTATIONS IMPROVEMENT
Readily accepts responsibility. Shows awareness of situation and environment by looking for better ways to do things. [Sees that something needs to be done and does it; or suggests other ways of doing things.] Always follows directions. Will ask for clarification when unsure. Absolute minimum of supervision. Work always turned in/done on time, and work area clean. Natural leader. Instills confidence in others. Cheerfully takes any job offered. Requires little supervision. Does not pass the buck but does not assume additional responsibilities or make helpful comments and suggestions. Believes in just getting by. Has very little drive. Never volunteers to undertake extra work. Resents change.
SELF-MOTIVATED
FOLLOWS DIRECTIONS
Carries out instructions well. Will ask if there are any questions. Usually completes assignments on time; .work area cleaned. Well thought of. Works with others in a team.
Usually accurate. Will sometimes ask if there is a question. Work completed, even if late; usually puts away materials after using them Uses average amount of tact and diplomacy.
Careless; makes recurrent errors. Does not ask for clarification of instructions. Some work completed, many turned in late. Sometimes forgets to clean work area. Inclined to be stubborn. Sometimes tactless.
COMPLETES TASK
Many assignments not completed. Work area messy. Arouses resentment and quarrels with others. Seldom gives others credit for ideas. Sleeps, loafs, talks or otherwise wastes working time. Causes distractions for others. Requires constant prompting.
Efficient and organized. When finished working, does extra work or does something else quietly.
Starts promptly and puts in a good days work. Does more than is required.

4-VERY GOOD
Seldom absent or late, unless for excellent reason. Almost always reports absences or tardies in advance. Makes up work time. Always listens and acknowledges criticism politely. Always polite and willing to help. Requires little supervision. Exact and precise most of the time.
3-GOOD
Occasionally absent or late. Does not always report absences or tardies in advance. Makes up some of the work time. Listens to advice given by others. Agreeable and pleasant.
2-NEEDS IMPROVEMENT
Occasionally absent and frequently late (or viseversa). Seldom makes up missed work time.
1-POOR
Often absent or late without good excuses. Does not make up missed work time.
ATTENDANCE/ PUNCTUALITY
Accepts criticism and makes an attempt to better the situation. Highly cooperative; cheerful; respected by others. Produces superior quality of work. Can be relied upon to always put out work which shows that time and effort has been spent on the assignment. Superior work output. Requires absolute minimum of supervisions
Becomes angry and resentful when given criticism. Not liked by others. Blunt; antagonistic. Poor quality of work. Shows that little/no time or effort has been taken. Makes frequent errors.
QUALITY OF WORK
Inclined to make mistakes. Displays a lack of pride in work appearance. Minimum effort.
