METHOD OF STATEMENT

Road Works, Drainage & R.C. Retaining Wall Bina Mahasil Sdn. Bhd.

1.0 Preamble 1.1 The work carried out by Bina Mahasil Sdn. Bhd. comprises infrastructure works, as detailed in the contract documentation. 1.2 This method statement covers laying of binder course and wearing course for operations by the Company which take place at Saujana Impian, Phase 6B. 2.0 Project Health, Safety and Environment 2.1 Copies of all relevant health and safety documentation will be retained on site by The Company for reference as necessary in accordance with the Occupational Safety and Health Act 1994(514) Section 16. 2.2 All workers shall be informed and attend the site safety meeting briefing by the Safety Officer from the CIDB. 2.3 Safety awareness of all site personal should be established through the safety awareness training from CIDB. It is essential for all site personal to hold the Green Card from CIDB. 2.4 The Company will make the following documents and information available on site, and ensure that they are applicable and relevant: (a) This method statement (b) Safety and Health Policy (c) Project Health, Safety and Environment Technical Guideline 3.0 Site Management 3.1 The Company will be represented on site during working hours by an experience foreman/supervisor. 3.2 The Company site supervisor will be responsible for ensuring that all works carried out by the Companys employees are carried out according to this Method Statement, Health & Safety legislation, the Companys health and safety requirements and their own safe systems of work. This will be achieved with the assistance of induction training and toolbox talks. 3.3 The site supervisor will ensure that all work equipment is checked and certified as appropriate. No work equipment will be used unless it is safe and suitable for its intended purpose.

Bhd. 4.0 Security 4.1 The Company will be responsible for securing the site against unauthorized entry outside working hours. 4.2 All equipment, tools and materials supplied by the company will be placed into safe storage whenever the site is to be left unattended. 4.3 Additional care will be taken by the company to ensure that loading, unloading and other hazardous operations do not take place in areas where other site contractors, members of the public and other non-construction personnel are present or may enter. 5.0 Machinery 5.1 Other than the machine operator, no person shall be allowed to ride on the machine, all machines shall be maintained in a good condition such that no premature failure of te brakes, steering and power train occur. Additionally all machines shall be equipped with a working horn and headlights, if night work is carried out. 5.2 No machine is to be overloaded so as to cause it to be overturned easily. 5.3 Machines at the site shall be serviced monthly, which includes oil & filter service. 5.4 The Company shall provide all necessary temporary plant and machineries to ensure smooth running of the job and to achieve the accepted date of completion, maintain them during the currency of the contract and remove at completion of work. 6.0 Emergency procedures 6.1 The evacuation/fire assembly point will be determined by the Company and advised to the site supervisor, who will ensure that this and all other health and safety information is passed to persons under his control. 6.2 Minor first aid will be attended to on site. A first aid kit will be kept at the site office. 6.3 Company employees will be instructed to report to the site supervisor if they suffer any accident or injury whilst at work.

Bhd. 7.0 Personal Protective Equipment 7.1 ALL SITE STAFF ARE TO WEAR SAFETY HELMETS on site except when in welfare accommodation. 7.2 Suitable safety footwear fitted with steel toe-protectors will be worn by all company personnel. Casual slippers and bare-feet will not be permitted.

8.0 Roadworks 8.1 Procedures for working on public roads and highways are contained in the company safety manual and will be adhered to at all times. 8.2 All road sign, barriers and cones will be regularly checked throughout the working day and as necessary. 8.3 Excavation for boxing out shall commence based on the required width and cross fall and design level. 8.4 Concrete kerb and scupper drains can proceed immediately after first layer crusher run has been laid. 8.5 Freshly completed road shall be prevented from being used for the next 3 days. 8.6 Road markings and signage shall be installed as required.

9.0 Sub-grade & Crusher-run laying 9.1 To peg out road alignment and island locations in accordance to Layout Plan issued by consultant. The alignment shall be cross-referenced with the Block Pegging. 9.2 To excavate to the required sub-grade level. To lay the crusher-run according to the specified thickness and using the specified bedding requirement as per construction drawings. Whereby unsuitable or soft sub-grade material occurs, it is to be excavated, replaced and backfilled with suitable material, and compacted to the specified requirements. 9.3 The works are to be carried out according to approved construction drawings and crosssections. Testing of the sub-grade and crusher-run layer and subject to contract specifications and requirements.

10.0

Binder and Wearing Course 10.1 Tack coat 10.1.1 To be applied over the prepared approved crusher-run base and/or binder course as specified. Average rate of application should be 0.15 to 0.35 kg/m 2. 10.1.2 Temperature should be between 600C to 900C curing time is min. 1 hour for K1-40. 10.1.3 Bleeding surface flow of bitumen (occurs due to over spray of tack coat) by placing hot mix over sprayed should be removed. 10.2 Asphalt Paving 10.2.1 A mixture of coarse aggregate, fine aggregate filler and bitumen binder. 10.2.2 Minimum required thickness is 50mm. Required minimum compaction is 95%. Batching temperature is around 1630C. Maximum laying temperature is min. 1350C to max. 1630C. Temperature in joints should be min. 900C. Asphalt should be applied at max. 1.50m to get required temperature in joints. 10.2.3 Rolling and paving of asphalt is carried out from lower levels to higher levels. To know number of passes required for minimum compaction, trail area of up to 30m should be prepared. In long transition joints between layers, 15cm offset is kept for good compaction. 10.2.4 Segregation: Accumulation of asphaltic aggregates should be minimized and replaced with fine mix. 10.2.5 Pot holes: Less asphalt or thin asphalt surface, poor drainage and ponding due to diesel and oil patches, slippage cracks may form. Cracks can be repaired by, in-situ sealant, hot mix asphalt emulsion slurry. 10.2.6 Camber: Two side slope (Nor. 1.5%), Cross Fall One-side slope (Nor. 2%). 10.2.7 Asphalt paving speed is between 4 to 6 km/min. Weight of steel tandom roller (STR) should be 20 to 25 tonnes. Weight of Pnuematic Tyre Roller (PTR) should be 20 to 25 tonnes, tyre pressure should be in between 6 to 6.3 kg/cm2 or 80 to 110 PSI. 10.2.8 Staggered rolling should be done in passes to avoid channeling. First and final rolling should be with STR and intermediate using PTR for optimum compaction.

Drainage Works 11.1 The setting out of the drainage shall be based on Pre-Computational Plan and carried out by Licensed Surveyor. The trench shall be excavated to the required level and checked before placing the base concrete. Before the base concrete had set the half-round drain shall be installed and the remaining mass concrete shall be filled beside the drain. Aggregate packing shall be placed on each weep hole of the drain. Sumps shall be constructed according to the locations required. The excavation materials shall be partly used for backfill and the remainder cart away.

Retaining Wall 12.1 The setting out of the retaining wall shall be based on Pre-Computational Plan and carried out by Licensed Surveyor. The trench shall be excavated to the required level and checked. Reinforcement arrangement in the wall section would be provided as shown in the drawing by lapping with previous bar arrangement. PVC pipes would be placed inside the reinforcement arrangement according to the spacing as specified in the construction drawings. The section of the wall to be concreted should be reasonably clean and free from soil and debris. Concrete slump testing shall be conducted by the standard slump measuring methods and the result shall confirm to the specified workability of the concrete mix. Concrete would be placed in successive layers and porker vibrator would be applied while concreting. Inlet of the weep hole would be enclosed by a suitable packing of geotextile wrapped surrounded with aggregates. Backfilling to commence when R.C. retaining wall has gained sufficient concrete strength. Compaction of backfill should be in successive layers with the application of vibrator roller, break compactors where necessary. 5

Alteration and Modification of Method Statement 13.1 Alterations and modifications to this Method Statement will be communicated to all employees and any other person who may be affected by those changes. No alterations or modifications to this Method Statement will be made without prior consultation with those affected.

Declaration This method statement is signed by a responsible representative of Bina Mahasil Sdn Bhd, who agrees to comply with all requirements of health and safety law, the entire contents of this method statement for this project and any instructions given by higher authorities or appointed safety advisors.
