In consideration of being allowed to participate in any way in the Wune Tang Academy Championships, the undersigned, the
parent or guardian of a minor child participant: 1. Agrees to inspect, prior to participation, the facilities and equipment to be used, and if the participant believes anything is unsafe, he or she should immediately withdraw from competition and refuse to participate. 2. Acknowledge and fully understand that each participant will be engaging in activities that involve risk of serious injury, including permanent injury and death, and severe social and economical losses which might result not only from their actions, inaction or negligence, but the action, inaction or negligence of others, the rules of play or the condition of the premises, or any equipment used. Further that there may be other risks not known to us or not reasonably foreseeable at this time. 3. Assume all the foregoing risk, and accept personal responsibility for the dangers following such injury, permanent disability, or death. 4. Released, waive, discharge, and convenant not to sue Wune Tang Academy, its affiliated clubs, the venue any and all sponsors, officials, promoters, instructors, or other participants, all of which are hereinafter referred to as releases, from any and all liability to each of the undersigned, his or her heirs and next of kin for any and all claims, demands, losses, damages on account of injury, including death or damage to property, caused or alleged to be caused in whole or in part by the negligence of the releases or otherwise. 5. TEAM AND CREATIVE COMPETITION REGISTRATION FORM CREATIVE SECTION APPLICATION: 10 PER ENTRY Wune Tang Academy will provide the music system but you MUST supply your own disk clearly labelled and ready to play. Creative section can be individual, due, trio or teams. You may incorporate gymnastics, acrobatics, dance and athletic disciplines. However at least half the form must originate from Martial Arts techniques. It must be choreographed in a general sense to the beats and rhythm of music chosen.
