Engineering & Maintenance Equipment Troubleshooting Report

(Load Break Switch Fuse Breakdown)

Problem Statement:
At 0810H during monitoring equipments; facilities heard chiller compressor equipment suddenly stopped, upon checking on site on chiller pad area; facilities saw chilled water pump #16 goes on fire. Facilities immediately suppress fire by spreading fire extinguisher and shut-off affected breakers main control center. Upon tracing the circuit line, equipment main circuit breaker (2500A) supplying central equipments was tripped and upon trying to reset the breaker it didnt energize. checking the utility supply voltage, facilities found out the main line experience single phasing (L1 = 496V, L2= 520V, L3=196V*) of plant utility line. Facilities instantly informed production (0820H) about the situation and call Sir RAA / IT personnel to relay the incident.

Actions Performed:
Facilities initiate one by one check-up of central equipment to assess its condition if it was affected by single phasing, other equipments was tripped on its overload contact but 3 units experience burnt winding (CHWP#16, COWP#01 & CHWP#05) Facilities operate the generator set to cater the load and restored in sequence related equipments. As of 1000H after securing load stability engineering informed production to resume their operation. Engineering called both MERALCO and SC Megaworld to report the situation and requested to check the load break switch of first private pole, as of 1020H both group arrived on the plant.

During checking of first private pole load break (phase A), contractor suggested to pull out the unit to see integrity of inside fuse. After disassembling, the fuse is already defective due to arcing of its coil component.

Load break switch was replaced by a new unit. As of 1600H the contractor completely replaced the busted fuse. After securing the main line is in good parameter condition, At 1645H facilities transfer power from genset to Meralco.

Root Cause Analysis:

Reviewing the failure fault mode encountered on utility line units, it is concluding that the single phasing of utility line was brought by busted fuse. Single phasing has been detrimental to ILIs 3 phase motors on operation due to its capability to damage its windings causing stoppage to operation.

Recommendations;

Consider to install interrupter on First private pole so that in any event of faults (Over/under voltage, Surging of voltage supply and main bus single phasing) it will disconnect the main line to prevent any damages on the unit.
Strict implementation of PMS to electrical lines to ensure and conform integrity of operation of each unit

Life cycle assessment of load break fuses. Provision of spare item would eliminate long downtime if in case of trouble.
